Position Summary:
We’re hiring an Attending Physician to provide consistent, relationship-driven care across assisted living and memory care communities. You’ll manage a stable patient panel, perform comprehensive assessments, and oversee the work of advanced practice providers who share your passion for person-centered medicine. Every visit matters—because every resident deserves care that’s both expert and empathetic.

Key Responsibilities:

• Conduct regular rounding visits and same-day acute care assessments
• Lead chronic disease management and medication optimization
• Collaborate with interdisciplinary teams on care coordination and treatment planning
• Mentor NPs and PAs, supporting their professional development
• Uphold the values of empathy, quality, and accountability in every encounter

Qualifications:

• MD or DO licensed in Minnesota
• Board certification in Internal Medicine, Family Medicine, or Geriatrics preferred
• Experience in long-term or post-acute settings a plus
• Excellent bedside manner, documentation skills, and team collaboration
• Current driver’s license and auto insurance
