Job Details:
The Supervisor of Social Worker, manages, monitors, and facilitates the work responsibilities for the department Social Workers. This will involve balancing clinical, strategic, regulatory, risk management and financial aspects in everyday decisions about care coordination, resource utilization and discharge planning as it relates to Social Work scope of service. The Supervisor of Social Worker’s role encompasses education of the Social Work team on the goals of the Care Coordination Department working collaboratively with other departments to achieve positive outcomes and a positive patient experience for all Mercy Hospital patients and families.
Responsibilities:

Establishes, manages, and evaluates day to day activities of the Social Work staff.

Maintains and oversees Social Work daily assignment to ensure adequate staffing on patient care units.

Processes staff time off requests and schedule.

Completes six week Social Worker staff schedule throughout the year

Able to use UKG for completion of staff payroll

Provides consistent and constructive feedback to Social Workers on their performance.

Completion of the annual Performance Evaluations for Social Work staff

Monitors and ensures appropriate documentation of all Social Work interventions.

Participates and coordinates in the hiring and orientation of newly hired Social Workers.

Serves as a resource to Social Work staff, physicians and hospital staff as needed and appropriate.

Collaborates with all disciplines and departments to optimize patient outcomes.

Represents the department on teams and committees as assigned

Provides input in development and restructure of department programs as assigned by Director of Care Coordination

Facilitates patient throughput positively affecting the continuum of care.

Monitors/Participates in interventions to decrease O/E..

Monitors employee performance with monthly random audit of charts for compliance and appropriateness of documentation and discharge planning.

Assignment to a specific unit as a working unit Social Worker.

Works collaboratively with the Supervisor of Managers of Care Coordination.

Updates department Policies and Procedures as assigned.
Qualification:
-
Graduate of an accredited College; current New York State Social Work License required, Masters in Social Work required and Licensed Clinical Social Worker (LCSW) preferred.

Prior supervisor experience preferred

Minimum of five years’ experience as a Social Worker in an acute hospital setting

Commitment to pursue ongoing education and self development to remain current with regulatory and industry standards and business objectives related to Social Work
Posted Salary Range: USD $49.48 - USD $61.84 /Hr. :
