Covenant House California (CHC) is a nonprofit organization that provides sanctuary and support for youth overcoming homelessness, ages 18-24. CHC believes that no young person deserves to be homeless; that every young person in California deserves housing, food, clothing, education, and most importantly, to be loved. CHC provides a full continuum of services to meet the physical, emotional, educational, vocational, and spiritual well-being of young people.

As this position provides direct care to young people that are 18-24 years old, we require that applicants are at least 25 years old.

POSITION SUMMARY:
Under the supervision of a Director, the Case Manager is responsible for provision of case management and custodial services within CHC. Essential functions of the position are client intake and assessment, case planning, documentation, and basic individual and group counseling. The position is also responsible for providing these services within the framework of the Mission Statement, Five Principles, and the Policies and Procedures of Covenant House California.
ESSENTIAL FUNCTIONS:
The goal at Covenant House California is to develop positive relationships with the residents through the effective utilization of the case management system. This system includes:

b. Full Assessment:
Review and integrate the Initial Background Interview (IBI) into the Individual Service Plan and the accessing of resources for the youth.
c. Individual Service Plan:
Engage in positive transition planning with the youth.
d. Documentation:
e. Case Review:
Prepare and present assigned cases at monthly Client Progress Review Meetings.
f. Group Facilitator:
Facilitate psycho-educational and support group activity with youth on a regular basis.
Provide custodial supervision as follows:
a. Punctuality and consistency for assigned shifts as well as all scheduled meetings, including transition, staff meetings, and training.
b. Perform duties as assigned to meet the needs of youth and ensure adherence to the daily program schedule. This includes, but is not limited to:
3. Other program duties as assigned by the Director. Such duties may include, but are not limited to the following program areas: HOPWA, ILP, HMIS, charts/chart room audits, or other specialized contract duties.
JOB REQUIREMENTS:
EDUCATION:
Bachelor’s degree in Psychology, Sociology, Social Work or related field OR an Associate’s degree in Psychology, Sociology, Social Work or related field and 2 years of case management experience, preferably with homeless or runaway youth.
EXPERIENCE:
In lieu of a Bachelor’s Degree, must have a minimum of 4 years of case management experience, preferably with homeless or runaway youth.
SKILLS:
Must be a self-starter and team player with the ability to work with other staff. Knowledge of adolescent development, community resources, and case management skills. Must have good organizational skills and the ability to communicate with members, clients, vendors and the public. Communication with and about youth must reflect a “strength based” and “trauma informed” approach. Bilingual in Spanish preferred but not required.
TRANSPORTATION:
Must have a valid CA Drivers License, reliable transportation and a good driving record.
WORKING CONDITIONS:
General business/office environment. Must be able to lift up to 40 lbs. on occasion, bend, sit and stand for prolonged periods of time. Some weekend/holiday work may be required. Full-time 40 hours per week.
SUPERVISORY DUTIES: This position does not perform supervisory duties
EQUIPMENT USED:
Must be able to utilize basic word processing, email, and internet programs. Must be able to communicate effective both orally and in written form.

The work environment characteristics described here are representative of those an employee encounters while performing the essential functions of this job. While performing the duties of this job, the employee may work occasionally in outside weather conditions. The noise level in the work environment is usually moderate to loud.
