Community Liaison

Alfa Home Health and Hospice is looking for a motivated Marketer/Community Liaison to represent the agency, build strong referral relationships, and grow our patient base in the home health and hospice arena.

Responsibilities:

- Develop and maintain relationships with hospitals, SNFs, ALFs, physicians, community groups

- Promote Alfa’s services and capabilities in home health and hospice

- Identify and cultivate new referral sources; maintain existing ones

- Conduct outreach presentations, facility visits, community events

- Coordinate with clinical teams to ensure seamless admissions

- Track referrals and growth metrics; report to leadership

Qualifications:

- Experience in healthcare marketing required (home health or hospice experience preferred)

- Strong communication, presentation and networking skills

- Self-motivated and able to meet growth targets

- Valid driver’s license and reliable transportation

Full-time position

Monday to Friday, 8:00 AM – 5:00 PM

Occasional evening or weekend community events as needed

Mileage reimbursement provided
