Pay range for this position is $37.05 - $54.54/hour. Actual pay is based on years of licensure. This is a Salaried position.
Overview:
POSITION SUMMARY: The Palliative Care Coordinator provides clinical support, education and development, case management and supervisory functions. The Coordinator ensures appropriate tracking of data and outcomes, runs Palliative Care Team meetings, serves as Palliative Care representative on Ministry Supportive Care, and enters required data. The Coordinator is also a Palliative Nurse and assists in responding to referrals, educating/mentoring nurses and other clinicians in evidence-based palliative care, assisting patients with post hospital discharge care planning, functioning as case manager for patients of OSF HealthCare Palliative Care Program. Provides care for patients and family/ significant others across the life span based on physical, psychosocial, educational, safety, and related criteria appropriate to the age of patients served in assigned service area. Rotates weekend call.
Qualifications:
REQUIRED QUALIFICATIONS:
- Bachelor’s degree in Nursing required within 6 years of hire
- Experience - Minimum of 3-5 years of clinical experience
- Hospice/Palliative Care Nursing Certification or to pursue certification within two years.
PREFERRED QUALIFICATIONS:
- Education - Baccalaureate degree in nursing is preferred
- Experience - CHPN Certification preferred
Licensure/Certifications • Currently Registered Professional Nurse in the State of Illinois
Other skills/knowledge • Excellent communication, organizational, and interpersonal skills • Demonstrated leadership ability and capable of priority setting • Strong knowledge of the nursing process and problem solving • Computer experience (word processing and data analysis) • Understanding of the principles of performance improvement, case management, discharge planning, utilization review, and medical records • Understands the basics workings of Joint Commission, DRG’s, and Medicare, Medicaid, and managed care reimbursement. Has a proven record of teamwork • Ability to work independently
