Hospice RN - PRN

We are seeking a Hospice RN to provide coverage in Victoria, Goliad, Beeville, and Refugio, Texas.

Summary

We are seeking a Registered Nurse Case Manager to join our team at White Orchid Hospice. In this role, you will be responsible for coordinating patient care, ensuring that our clients receive personalized and compassionate support throughout their hospice journey. Your expertise will play a vital role in enhancing the quality of life for our patients and their families.

Responsibilities

• Coordinate and manage comprehensive patient care plans for hospice patients.
• Conduct thorough assessments of patient needs and develop individualized care strategies.
• Collaborate with interdisciplinary teams, including physicians, pharmacists, and mental health professionals.
• Monitor patient progress and adjust care plans as necessary to ensure optimal outcomes.
• Provide education and support to patients and families regarding hospice services and end-of-life issues.
• Maintain accurate medical records and documentation in compliance with regulatory standards.

Requirements

• Current Registered Nurse (RN) license in the state of Texas.
• Experience in hospice or palliative care required.
• Strong patient care skills with a compassionate approach.
• Excellent communication and interpersonal skills.
• Ability to work collaboratively within a multidisciplinary team.
• Familiarity with medical records management and utilization management processes.
