Position Highlights

• Full-time or part-time availability
• No nights, weekends, or call
• “Round and Go” model – manage your time and patient load independently
• Collaborate with interdisciplinary teams to optimize care
• Manage new admissions and provide ongoing care for a geriatric population
• Participate in facility quality improvement initiatives and care coordination

Requirements

• MD or DO, Board Certified in Internal or Family Medicine
• Current, unrestricted state medical license and DEA registration
• Strong clinical and interpersonal skills
• Proficiency with EMR systems
• Passion for post-acute medicine and team-based care
