Job Summary

We are looking for a compassionate and detail-oriented Registered Nurse (RN) Care Coordinator to oversee caregiver supervision, develop and manage individualized care plans, and maintain strong relationships with clients and their families. This role plays a key part in ensuring compliance with agency standards and adherence to all guidelines outlined in the RN Care Coordinator Manual.

Qualifications

• Active RN license (required)
• At least 2 years of nursing experience
• Background in geriatric care is a plus
• Excellent communication, documentation, leadership, and organizational skills
• Professional attitude with the ability to collaborate effectively with clients and team members

Key Responsibilities

• Perform initial in-home assessments and conduct ongoing client evaluations
• Develop, implement, and regularly update care plans, including 90-day reassessments
• Act as the main point of contact for assigned clients and their families
• Supervise caregivers, provide guidance, and document performance
• Track patient conditions and record any changes or progress
• Educate clients and caregivers on health conditions, medications, and home care practices
• Coordinate care services in alignment with agency policies
• Report any changes in patient status to the Director of Nursing (DON)
• Maintain thorough and accurate clinical documentation
• Support caregiver onboarding, orientation, and training as needed
• Work collaboratively with RN team members to provide every-other-weekend coverage

Schedule Expectations

• Primarily Monday through Friday, 9:00 AM – 5:00 PM
• On-call communication availability every other weekend
