Job Description:
A committed and compassionate Licensed Practical Nurse (LPN) is needed to provide high-quality nursing care within a skilled nursing environment. The LPN will support residents with daily clinical needs, ensuring safety, comfort, and adherence to care standards.

Responsibilities:

• Provide routine nursing care in alignment with established care plans and facility guidelines.

• Administer medications and treatments as prescribed, ensuring accurate documentation.

• Monitor patient conditions, record vital signs, and promptly report changes to supervising RN or provider.

• Assist with wound care, catheter care, and other clinical procedures based on competency and scope.

• Support residents with ADLs when needed to promote comfort and well-being.

• Maintain accurate and timely charting in compliance with facility and regulatory standards.

• Collaborate with an interdisciplinary team—including RNs, therapists, CNAs, and providers—to ensure coordinated patient care.

• Educate residents and families on care routines, medication administration, and safety practices.

• Ensure a clean, safe, and supportive environment for all residents.

• Uphold all state, federal, and facility regulations governing nursing practice.

Qualifications:

• Active LPN license in the state of New Mexico (or ability to obtain prior to start).

• Graduate of an accredited Practical Nursing program.

• Experience in skilled nursing or long-term care strongly preferred.

• Strong communication, clinical assessment, and documentation skills.

• Ability to work collaboratively and deliver patient-centered care.
