Position Summary:
As a PRN LPN, you will conduct supervisory visits in accordance with state regulations and company policy. These visits are essential to maintaining high standards of care and supporting our field staff.


Responsibilities:

  • Conduct supervisory visits for home health aides (HHAs) and personal care aides (PCAs) in client homes

  • Evaluate and document performance and adherence to care plans

  • Ensure clients’ needs are being met and report changes to the RN or case manager

  • Educate aides as needed during supervisory visits

  • Complete all required documentation accurately and timely

  • Maintain regular communication with clinical supervisors and office staff.

    Qualifications:

    • Current and valid LPN license in the state of Georgia

    • CPR and First Aid Certification

    • Minimum of [1–2] years of clinical experience, preferably in home health or community-based care

    • Reliable transportation and valid driver’s license

    • Strong communication and observation skills

    • Ability to work independently and manage a flexible schedule
