The Position:

We are looking for a full-time LPN Assessment & Triage Nurse to work with our team of health professionals to provide Chronic Care Management to Medicare patients nationwide. The LPN will help lead a team of Care Coordinators who call patients monthly to help them manage their conditions. The Nurse will review the patient’s care plan for ongoing updates, triage any patient escalations, verify the accuracy of completed assessments, clinic correspondence, etc.
