LPN (Licensed Practical Nurse) - Private Duty Nurse

Job Summary:

Independent Options, LLC is seeking a compassionate and reliable Licensed Practical Nurse (LPN) to provide nursing support and healthcare coordination for individuals with Intellectual and Developmental Disabilities (IDD) in community and residential settings. The ideal candidate will be passionate about person-centered care and committed to supporting individuals in achieving the highest quality of life possible.

Responsibilities:

• Provide direct nursing care and health monitoring for individuals receiving services
• Administer medications and treatments as prescribed by physicians
• Monitor vital signs, medical conditions, and overall health status
• Communicate with physicians, pharmacies, families, and interdisciplinary teams regarding medical needs and concerns
• Complete nursing documentation and reports in accordance with company policy and ODP regulations
• Assist with medical appointments and follow-up care as needed
• Ensure compliance with medication administration procedures and infection control practices
• Promote dignity, independence, and person-centered supports for all individuals served

Qualifications:

• Current Pennsylvania LPN license in good standing
• Minimum of one (1) year nursing experience preferred
• Experience working with individuals with IDD, behavioral health, or community-based services preferred
• Strong communication, organizational, and documentation skills
• Ability to work independently and as part of a multidisciplinary team
• Valid driver’s license and reliable transportation
