LPN-Pennsylvania Licensed Practical Nurse required

Duties

  • Perform comprehensive patient assessments, including vital signs, physical examinations, and physiological evaluations to inform individualized care plans.
  • Administer medications and injections accurately, including IV infusions and botulinum toxin treatments, following strict protocols and HIPAA regulations.
  • Manage airway support and ventilator care for patients requiring respiratory assistance, ensuring optimal functioning and safety.
  • Conduct diagnostic evaluations such as specimen collection, processing, and basic medical imaging procedures to support clinical decision-making.
  • Provide specialized dementia and Alzheimer's care, including memory care activities and behavioral health management tailored to patients with cognitive impairments.
  • Assist with complex procedures like catheterization, suturing, spinal taps, and gastric lavage while maintaining aseptic techniques.
  • Document all patient interactions thoroughly using EMR (Electronic Medical Records) systems such as Cerner or Athenahealth; ensure compliance with HIPAA standards.
  • Collaborate with interdisciplinary teams in acute care environments like Level I Trauma Centers or Critical Care Units to deliver timely interventions during emergencies or triage situations.
  • Support discharge planning and case management efforts to facilitate smooth transitions from hospital to home or community-based care settings.
  • Engage in health coaching and patient education initiatives focused on geriatrics, pediatrics, or chronic disease management to promote long-term wellness.

Experience

  • Proven experience working in diverse healthcare environments including hospitals (ICU, ER), nursing homes, assisted living facilities, outpatient clinics, or home care settings.
  • Critical care experience with Level I Trauma Centers or Level II Trauma Centers preferred; familiarity with ICU protocols is highly valued.
  • Hands-on experience with EMR/EHR systems such as Cerner or Athenahealth for accurate medical documentation and coding (ICD-9/10; CPT).
  • Demonstrated ability to perform IV insertions, phlebotomy (venipuncture), medication administration, and sterile processing techniques confidently.
  • Knowledge of medical terminology across specialties such as cardiology (cardiac catheterization), ophthalmology (dermal fillers), neonatology (neonatal ventilators), and infectious disease protocols.
  • Experience working with individuals with disabilities or developmental disabilities; familiarity with dementia/dementia-related conditions like Alzheimer’s disease is advantageous.
  • Strong understanding of physiology, anatomy, pharmacology related to pain management, hospice care, and acute/chronic illness management.
  • Certification in Basic Life Support (BLS) required; additional certifications such as ACLS or PALS are a plus for advanced practice environments.
