LPN/CMA Care Coordinator

AuthoraCare Collective is seeking a compassionate and organized Care Coordinator to support patients in navigating their healthcare journey. In this role, you will coordinate patient care by assessing needs, developing care plans, and communicating with patients, caregivers, and healthcare providers to ensure individuals receive the services and support needed for the best health outcomes.

Care Coordinators build strong relationships with patients through regular outreach and support programs such as Chronic Care Management (CCM), Behavioral Health Integration (BHI), Principal Care Management (PCM), and Remote Patient Monitoring (RPM). This role also includes gathering information from electronic medical record (EMR) systems and helping identify gaps in care.

The ideal candidate has strong communication skills, is comfortable working with multiple EHR systems, and can think critically in a fast-paced healthcare environment. Join our team and help support successful aging in place.

This position is Full-time: Monday - Friday 8:00am - 5:00pm supporting all of our service areas. Office base/High Point.

Education and Experience:

  • Graduate of an LPN or Medical Assistant Program (CMA preferred)
  • Minimum of two (2) years’ experience in care coordination
  • Must encompass compassion, resourcefulness, and be a team player
  • Familiarity with electronic medical records (EMR) systems
  • Must complete and pass HIPAA compliance education courses
  • Must have the ability to multitask and manage time effectively
  • Excellent written and verbal communication skills
  • Outstanding problem solving, attention to detail, and organizational skills
  • Microsoft Office Suite experience
  • Other: Valid state-issued driver's license required. Must carry automobile liability insurance at limits required by agency. Must have own transportation.

Key Responsibilities:

  • Conducting patient assessment to identify patient needs and goals
  • Developing and implementing comprehensive care plans
  • Communicating effectively with patients, families, and healthcare team members
  • Educating patients about their conditions, treatment options, and medication management
  • Facilitating referrals to community resources and support services
  • Monitoring patient progress and making adjustments to care plans, as needed
  • Performing check-in calls for any appropriate problems and relaying concerns to providers and staff
  • Advocating for patients to ensure they receive appropriate care
  • Maintaining patient records and documentation
  • Participate in training new care coordinators, as needed
  • Data Entry: Input data related to medical information and other healthcare information into computer systems.
  • Review Records: Review medical records for treatment plans, problem lists, medication reconciliation, etc.
  • Quality Assurance: Regularly cross-reference and verify data across different sources to maintain high data quality.
  • Communication with Healthcare Providers: Work closely with healthcare providers to ensure issues and/or high-risk patient care is communicated effectively.
  • Compliance Monitoring: Maintain adherence to HIPAA guidelines and regulations to maintain ethical standards and avoid legal consequences.
  • Continuing Education: Keep up to date with changes in care management standards and participate in educational opportunities to improve job performance.
