About the Role

Seeking a dedicated MSW (Medical Social Worker) to provide patient counseling, coordinate home health services, and support both community and clinical operations in our Family Clinic network. This role is key in delivering compassionate psychosocial care and seamless service integration for patients and families.

Key Responsibilities

• Provide patient counseling and psychosocial support as part of routine clinical care and during acute events.​
• Coordinate home health services, including scheduling nurse and phlebotomy visits, maintaining home visit records, and ensuring timely follow-ups.​
• Assist doctors and nurses in ensuring continuity of care and holistic patient support throughout the care journey.​
• Facilitate communication between patients, families, and healthcare teams while supporting overall clinic operations.​
• Guide patients and families to available community resources for financial, emotional, or social support as needed.​
• Maintain accurate documentation of patient interactions and home health activities in compliance with clinic protocols.​
• Participate in team meetings, training, and case conferences as required.​
• Provide crisis intervention and advocate for patients’ rights and access to care.​
• Support adult safeguarding, discharge planning, and linkage with statutory/community agencies as needed.​

Qualifications

• Master’s Degree in Social Work (MSW) – Medical & Psychiatry specialization is preferred.​
• Strong communication, interpersonal, and coordination skills.​
• Empathy, patience, and cultural sensitivity are essential for the role.​
• Computer proficiency for basic case documentation.
