Nurse Liaison

JOB SUMMARY

Serves as liaison to coordinate and link hospital resources, acts as interface with third party payors/home health, coordinate research projects and to provide adequate follow-up and rehabilitation for patient; also serves as clinical liaison to identify compliance/quality improvement problems. Provides professional nursing services according to established standards and practices.

Key Job Responsibilities

Essential Function

Attend rounds discussing patient plan with medical staff; maintains treatment protocols and monitors for compliance and results.

Facilitates the timely and appropriate completion of medical records and forms; arranges for monthly inpatient chart review.

Maintains follow-up and coordinates with patient/family/hospital departments regarding all aspects of patient care needs (i.e., patient transfer, discharge planning, rehab services, outpatient visits).

Provides professional nursing care in accordance with established hospital/ department policies and procedures. Demonstrates knowledge and skills to provide care and meet age-specific patient needs.

Interacts and communicates in a customer responsive and professional manner. Works collaboratively and coordinates activities with hospital/medical staff and community agencies to integrate and implement activities.

Participates in monitoring/reviewing quality improvement and utilization review activities. Serves as contact person regarding all inquiries and reports.

Prepares and maintains required statistics, reports, records and files.

Coordinates research projects.

Participates on various nursing and hospital committees.
