Supportive Care Specialist (SCS) is dedicated to enhancing the quality of life for patients living

with serious or life-limiting illnesses across the Seattle region. We provide

compassionate, patient-centered palliative care within Long-Term Acute Care (LTAC) and

acute care facilities. Our mission is to integrate clinical expertise with empathy—helping patients

and families navigate complex healthcare decisions while improving comfort, communication,

and dignity.

burden. This position offers autonomy, flexibility, and the opportunity to provide expert care to

patients with complex medical needs in LTAC and acute care settings.

Position Overview

• SCS Physicians collaborate closely with a dedicated Nurse Practitioner (NP) and a

multidisciplinary care team to develop personalized treatment plans, manage symptoms,

and ensure coordinated care transitions. This role is ideal for physicians who value

patient connection, clinical independence, and a balanced work-life schedule.

• All clinical decisions, care coordination, and referrals must be based solely on

independent medical judgment and the best interests of the patient. Supportive Care

Specialist strictly adheres to Anti-Kickback Statute (AKS) and Stark Lawcompliance.

Compensation and incentives are never tied to the volume or value of referrals or

federally reimbursable services.

Primary Responsibilities

Duties include but are not limited to:

• Lead clinical care for patients in LTAC and acute care facilities, guiding medical

decision-making and supporting families through complex care transitions.

• Manage symptoms related to serious illness, including medication management and

palliative interventions, in collaboration with the NP.

• Conduct daily rounds with NP support to optimize patient follow-up, documentation, and

interdisciplinary communication.

• Develop proactive care plans tailored to patient goals, safety, and long-term well-being.
• Partner with specialists (e.g., oncologists, cardiologists, pulmonologists) and primary care

providers to ensure continuity and coordination of care.

• Facilitate discussions regarding treatment options, goals of care, and advance care

planning.

• Accurately document all clinical encounters in the Electronic Health Record (EHR).
• Demonstrate compassion, emotional intelligence, and a commitment to patient-centered

care.

• Collaborate effectively within the interdisciplinary team to promote evidence-based

palliative practices.

Qualifications

• Doctor of Medicine (MD) or Doctor of Osteopathic Medicine (DO) from an accredited

institution.

• Board Certified or Board Eligible in Palliative Care (preferred) or related specialties such

as Internal Medicine, Family Medicine, or Geriatrics.

• Active and unrestricted state medical license.
• Active DEA license.
• Experience in acute care or LTAC settings preferred.
• Strong interpersonal and communication skills, with a compassionate, patient-centered

approach.
