Primary Care Provider (MD/DO)

For over 30 years, Janian has been a leader in developing and implementing person-centered, community-based care for New Yorkers with lived experience of homelessness. We are committed to a manageable patient caseload with a focus on quality over quantity with protected administrative time within the workday to complete documentation. We also have a weekly protected half day for professional development including an accredited grand rounds series. Our clinics are co-located with psychiatry and case management with close collaboration. We continuously strive to create a practice that nurtures therapeutic relationships, supports evidence-based care, and grapples with the social and economic forces that lead to inequities and disparities in service and outcomes. Janian’s primary care program, started over 10 years ago, continues to expand rapidly. Janian Primary Care Providers provide quality primary care in a variety of settings including permanent supportive housing, transitional shelters, safe havens, and drop-in centers, as well as within the community through Street Medicine. Primary Care Providers work closely with supportive staff as well as collaborate with behavioral health providers, social workers and case managers at program sites and residences, and street outreach teams who identify and refer medically vulnerable clients.
