Registered Nurse - Home Health

Job Overview
URGENTLY seeking a compassionate and dedicated Registered Nurse to join our Home Health team. In this role, you will provide high-quality nursing care to patients in their homes, ensuring their comfort and well-being. You will work collaboratively with a multidisciplinary team to develop and implement individualized care plans, focusing on improving patient outcomes and enhancing their quality of life.

CLIENT IS LOCATED IN DENVER. AVAILABLE SHIFTS ARE MONDAY TO SUNDAY MORNING AND EVENING VISITS

Duties

• Conduct comprehensive assessments of patients' health status and needs in the home setting.
• Develop, implement, and evaluate individualized care plans based on patient assessments and goals.
• Administer medications and treatments as prescribed, including IV infusions and pain management interventions.
• Provide education to patients and families regarding disease management, medication administration, and home care practices.
• Collaborate with healthcare professionals to coordinate discharge planning and ensure continuity of care.
• Maintain accurate medical documentation in compliance with regulatory standards and organizational policies.

Requirements

• Current Registered Nurse (RN) license in Colorado
• Current CPR Certification
• Current TB Test
• Previous experience in home health or community care preferred.
• Proficiency in medical documentation practices, including familiarity with Alora or similar electronic health record systems.
• Strong knowledge of discharge planning processes and pain management strategies.
• Ability to perform IV infusions safely and effectively.
• Excellent communication skills, both verbal and written, with a focus on patient education.
• Compassionate demeanor with a commitment to providing high-quality patient care in a home environment.

Job Types: Full-time, Part-time, Travel nursing, PRN

Pay: $50.00 - $60.00 per hour
