</p><p><b>Position Overview:
</b></p><p>As a Home Health Physical Therapist, you will play a crucial role in providing specialized rehabilitation services to our patients within the familiar and supportive environment of their homes. You will work closely with patients to assess their physical condition, develop personalized treatment plans, and implement therapeutic exercises to promote recovery, improve mobility, and enhance overall well-being.
</p><p><b>This is a PRN position (NOT FULL-TIME), meaning you will be working with us on an as-needed basis.
</b></p><p><b>You will be hired as a W2 employee, not as an independent contractor.
</b></p><p><b>We offer flexible scheduling, and you can freely choose the areas you prefer to cover.

</p><p><b>Key Responsibilities:
</b></p><div><ul><li>Conduct thorough assessments of patients' physical conditions to determine rehabilitation needs.
</li></ul><ul><li>Develop and implement individualized treatment plans in collaboration with the healthcare team.
</li></ul><ul><li>Provide hands-on physical therapy interventions to improve patients' strength, flexibility, balance, and coordination.
</li></ul><ul><li>Educate patients and their families on therapeutic exercises, mobility aids, and strategies to promote independent living.
</li></ul><ul><li>Monitor and document patients' progress, adjusting treatment plans as necessary.
</li></ul><ul><li>Collaborate with other healthcare professionals, including physicians, nurses, and occupational therapists, to ensure comprehensive patient care.
</li></ul><ul><li>Maintain accurate and up-to-date patient records
</li></ul><ul><li>Communicate effectively with patients, families, and caregivers to ensure a positive and supportive therapeutic environment.
</li></ul></div><p><b>Qualifications:
</b></p><div><ul><li>Current state licensure as a Physical Therapist.
</li></ul><ul><li>Prior experience in home health is preferred, but not required
</li></ul><ul><li>Strong clinical and critical thinking skills.
</li></ul><ul><li>Excellent communication and interpersonal abilities.
</li></ul><ul><li>Ability to work independently and as part of a collaborative healthcare team.
</li></ul><ul><li>Proficient in Microsoft Office Suite and comfortable using technology and electronic documentation systems.</li></ul></div></div></div></div>
