Responsibilities<div><br>
<div><ul><li><p>Verify plan of care with the RN prior to each visit.
</p></li><li>Collaborate with physician and RN frequently to ensure patient needs are being met; Notify them as soon as possible when patient condition changes.
</li><li>Teach patients and caregivers self-care techniques.
</li><li>Check vital signs.
</li><li>Review all medications and perform a medication reconciliation at each visit.
</li><li>Perform specialized procedures ordered by physician ( i.e. foley catheter change).
</li><li>Prepare equipment and materials for treatments using the aseptic technique.
</li>Complete documentation timely and accurately.<br>

Qualifications<br>

<div><div><ul><li>One year of clinical LPN experience.
</li><li>Current LPN license, specific to the state(s) you are assigned to work.
</li><li>Current CPR certification.
</li><li>Valid driver's license, reliable transportation and liability insurance.

Responsibilities<div><br>
<div><ul><li><p>Coordinates and oversees all aspects of patients' home care.
</p></li><li>Collaborates with patient care teams to develop and implement care plans.
</li><li>Educates families on patient condition, needs and care.
</li><li>Performs head to toe physical assessments.
</li><li>Checks vital signs and manages medications.
</li><li>Gives injections, IV therapy or tube feedings.
</li><li>Administers and maintains catheters.
</li><li>Completes documentation timely and accurately.
</li><li>Supervises LPNs and HHAs.
</li><li>Regularly communicates patient progress to the clinical manager, physician and care team.
</li>Other duties as assigned.<br>

<p></p>Qualifications<br>

<div><div><ul><li>One year of clinical experience as a registered nurse (RN). If less than 1 year clinical experience as a RN, candidate must be approved by VP clinical.*
</li><li>Current RN license, specific to the state(s) you are assigned to work.
</li><li>Minimum six months home health experience.
</li><li>Current CPR certification.
</li><li>Valid driver's license, reliable transportation and liability insurance.
</li><li>Able to assess patient status and identify requirements relative to age specific needs.
</li><li>Must be familiar with general use and functions of the computer, such as, user names and password concepts; internet; e-mail; navigation of computer desktop, including starting programs, using files and windows, effectively use navigation buttons and tool bars; ability to self-manage online HR services and online training programs.
</li></ul><div></div><ul><li>Note - If less than 6 months clinical experience as a RN, candidate must participate in RN Intern program.
