Summary of this position:

The Admissions Nurse is responsible for determining a patient&rsquo;s eligibility for hospice services. This position will review medical records, complete in person evaluations and communicate with the Medical Director to determine a patient&rsquo;s initial plan of care.

Job duties of this rewarding position include:
<ul>
<li>Completes an initial comprehensive assessment of patient/client and family to determine hospice needs.</li>
<li>Provides a complete physical assessment and history of current and previous illness(es).</li>
<li>Responsible for setting up DME and medications.</li>
<li>Provides professional and direct nursing care by utilizing all elements of nursing process and as defined by the State Nurse Practice Act.</li>
<li>Counsels the patient/client and family in meeting nursing and related needs.</li>
<li>Provides health care instructions to the patient/client as appropriate per assessment and plan of care/nursing diagnosis.</li>
<li>Communicates with the physician regarding the patient&rsquo;s/client&rsquo;s needs and reports any changes in the patient&rsquo;s/client&rsquo;s condition; obtains/receives physician&rsquo;s orders as required.</li>
<li>Ensures that arrangements for equipment and other necessary items and services are available.</li>
<li>Provides education to patients and families regarding available hospice services.</li>
<li>Demonstrates C.A.R.E. values to our patients/clients, family members, customers and staff.</li>
<li>Complies with the company&rsquo;s privacy practices/procedures and all state/federal privacy laws as outlined by HIPAA related to patient/client and employee records, to include but not limited to information accessed through any company web-based system and/or electronic medical records.</li>
<li>Demonstrates teamwork and prompt and regular attendance to work to ensure that quality care and services are provided to the patients/clients we serve.</li>
<li>Complies with and adheres to the appropriate use of Personal Protective Equipment (PPE) required by the Bloodborne Pathogens Standards. Protective Personal Equipment (PPE), including personal protective equipment for eyes, face, extremities, protective clothing, and protective shield and barriers, will be provided, used, and maintained.</li> 
</ul>
Job Requirements:
<ul>
<li>Graduate of an accredited school of nursing. One (1) to two (2) years of recent acute care experience in an institutional setting.</li>
<li>Current Registered Nurse licensure in state and CPR certification.</li>
<li>Bachelor's degree, with one (1) year of home health care experience preferred.</li>
<li>Management experience not required. Responsible for supervising home health aides.</li>
