The ideal candidate will possess a strong background in patient care, with experience supporting individuals with diverse needs, including those with developmental disabilities, dementia, or requiring hospice care. This role offers an opportunity to make a meaningful difference in the lives of our clients by providing attentive assistance with daily activities and health monitoring.Responsibilities<ul><li>Assist clients with Activities of Daily Living (ADLs) such as bathing, dressing, grooming, and mobility support.</li><li>Monitor and observe patients&rsquo; health status, including vital signs and overall well-being.</li><li>Support medication administration and ensure proper documentation in electronic health record (EHR) systems while maintaining HIPAA compliance.</li><li>Provide specialized care for individuals with dementia or Alzheimer's disease, including memory care techniques.</li><li>Operate medical equipment such as Hoyer lifts, catheters, and other assistive devices safely and effectively.</li><li>Assist with meal preparation, feeding, light cleaning, laundry, and maintaining a clean living environment.</li><li>Document patient progress accurately in medical records and communicate effectively with healthcare teams.</li><li>Follow all safety protocols during patient care activities and participate in ongoing training such as CPR certification.</li></ul>Skills<ul><li>Strong patient observation skills to detect changes in health status promptly.</li><li>Experience working in assisted living facilities, nursing homes, or home health environments.</li><li>Ability to assist with complex tasks such as catheter care, hospice support, and dementia-specific needs.</li><li>Competence in vital signs measurement and medication administration procedures.</li><li>Excellent communication skills to interact empathetically with clients and their families.</li><li>Prior long-term care experience working with individuals with disabilities or developmental challenges is preferred.
