The RN Navigator provides essential support to patients by guiding them through the healthcare system, ensuring they have access to necessary resources, and helping them manage all aspects of their care journey. This role involves assisting patients in understanding their medical condition(s) and treatment plans, including assisting patients with scheduling, overcoming logistical barriers, and connecting them with community resources to ensure a smooth and supportive healthcare experience. Navigators may work in a specific specialty area (ex: oncology, women&rsquo;s/breast health, cardiology, neuro, etc.) based on organizational/patient needs and clinical background, to provide focused patient navigation support.
RN Navigator &ndash; St. Francis Downtown - Full-time
Essential Functions:<ul><li>
Guides patients in understanding their diagnoses, treatment options, and the healthcare process, including developing patient education programs and tools.</li><li>
Serves as the essential link for communication and assessment between patients and all care providers.</li><li>
Coordinates and advocates for care across multidisciplinary teams, ensuring that all medical, social, and psychological needs are addressed. This may involve scheduling multidisciplinary planning conferences, developing concise patient summaries for use by the care team, and documenting recommendations using standardized protocols in accordance with nationally recognized care guidelines.</li><li>
Monitors patient progress and communicates with the care team to adjust treatment plans as necessary.</li><li>
Facilitates appointments for consultations and support services within established service standards, including social workers, dietitians, physicians, etc.</li><li>
Participates in patient education programs, community outreach, and quality improvement initiatives.</li><li>
Tracks patient outcomes, ensuring findings are communicated to patient/providers. May also prepare reports to support quality initiatives and programs.</li></ul>

Education:
Bachelor&rsquo;s degree in nursing (preferred)
Licensure/Certification:<ul><li>
Licensed Registered Nurse in state of practice (required)</li><li>
BLS Basic Life Support &ndash; American Heart Association (preferred upon hire, must obtain from approved source prior to direct patient care at BSMH; required at hire for RSFH)</li><li>
Oncology Nurse Navigator &ndash; Certified Generalist (ONN-CG) &ndash; Academy of Oncology Nurse &amp; Patient Navigators (preferred, if placed in Oncology department)</li><li>
Breast Health Clinical Navigator (BHCN) &ndash; National Consortium of Breast Centers (NCBC) (preferred, if placed in Mammography department)</li></ul>
Experience:
3 years of direct patient care experience as an RN (required)
Experience with discharge planning or utilization review (preferred)

Patient Services Representative &ndash; Airline Boulevard Medical Associates
Job Summary:
The Patient Services Representative is the first line of quality service to our patients and the community. This position will be responsible for processing patient registration, verifying demographics, obtaining insurance cards, and patient identification. Responsibilities include scheduling appointments, transcribing orders, explaining financial options to patients, and updating medical records accurately and efficiently. This position will provide excellent customer service and may be asked to occasionally cover other physician practice locations as needed.
Essential Functions:<ul><li>
Serves as the primary point of contact between patients and physician practices</li><li>
Provides strong communication and excellent customer service skills by greeting patients and the community in a respectful manner</li><li>
Answers internal and external calls in a friendly and helpful manner, routes calls, schedules patients, and enters necessary information for patient scheduling into the computer system in a timely and accurate manner.</li><li>
Processes patients in practice as they present for their appointments.</li><li>
Possesses the ability to troubleshoot and resolve problems promptly, ensuring patient flow is maintained and informs supervisor of any department and patient issues immediately</li><li>
Processes admission paperwork, including basic insurance verification. Secures, completes and verifies all pertinent patient demographic and insurance information as part of the registration process., Corrects registration errors as needed.</li><li>
Records time indicators for lobby wait times.</li><li>
 Calculates patient liabilities and actively collects and processes patient payments.</li><li>
Reconciles cash drawer at the close of the day.</li><li>
Performs charge entry for external services (i.e. nursing homes) as necessary.</li><li>
Schedules referrals or follow-up appointments and/or assists with scheduling, rescheduling or canceling other services for patients. Assists patients with online scheduling and MyChart as necessary.</li><li>
Assists patients in obtaining necessary referrals for follow-up services and record referrals on tracking tool (referral/consults).</li><li>
Responsible for and/or assist in obtaining proper authorizations and pre-certifications if applicable for all procedures scheduled through the physician practice</li><li>
Pulls patient charts as needed per office policy, files and maintains information as appropriate in chart per policy, and routes charts to clinical staff as indicated in office per policy</li><li>
Verifies RX benefits in electronic health record, per protocol</li><li>
Refers patients to financial counselors when additional financial counseling or payment arrangements are needed. Completes accounts in revenue cycle software</li></ul>
