Job Overview We are seeking a compassionate and highly organised Patient Care Coordinator to join our dedicated team. This is a Full-Time, remote position. In this vital role, you will be instrumental in supporting our mission to improve academic achievement, build character, and strengthen life skills for the youth we serve. You will act as a central point of contact, ensuring our members and their families have access to the resources and support necessary to thrive.  Responsibilities As a Patient Care Coordinator, your primary duties will include:  <ul><li>Serving as the main liaison between club members, their families, and internal or external support services.</li><li>Conducting initial assessments to understand the unique needs of each child and family, focusing on their health, well-being, and developmental goals.</li><li>Developing, implementing, and managing individualised support plans in collaboration with programme staff and families.</li><li>Coordinating with local community partners, healthcare providers, and social service agencies to connect families with essential resources.</li><li>Maintaining accurate, confidential, and up-to-date records of all interactions, care plans, and progress notes.</li><li>Proactively monitoring the well-being of members and providing ongoing support and guidance to their families.</li><li>Communicating effectively with all stakeholders to ensure a seamless and supportive experience for everyone involved in a member's care.</li><li>Championing the vision of our organisation, helping ensure every young person has a plan for their future and leads a healthy lifestyle.</li></ul>Qualifications The ideal candidate will possess the following skills and experience:  <ul><li>Proven experience in a similar role, such as care coordination, case management, or social work, preferably within a youth-focused environment.</li><li>Exceptional interpersonal and communication skills, with a natural ability to build rapport and trust with children and adults from diverse backgrounds.</li><li>Strong organisational and time-management skills, with the ability to manage a varied caseload independently from a remote setting.</li><li>A deep sense of empathy, patience, and a genuine passion for youth development and community support.</li><li>Proficiency in standard office software and experience with case management or client relationship management (CRM) systems.</li><li>A comprehensive understanding of local community resources available in the El Paso area is highly advantageous.</li><li>A degree in Social Work, Psychology, Public Health, or a related field is desirable but not essential.</li></ul>

We are seeking a dedicated and customer-focused IT Help Desk Analyst to join our team. The successful candidate will be the first point of contact for our staff, providing essential technical assistance and support for all IT-related issues. Your role is crucial in ensuring our team has the reliable technology needed to effectively serve the youth in our community. This is an excellent opportunity for a skilled IT professional who is passionate about using their expertise to support a valuable mission. Key Responsibilities - Serve as the primary point of contact for technical support requests from staff, received via phone, email, or our internal ticketing system. - Diagnose, troubleshoot, and resolve hardware, software, network, and peripheral issues in a timely and professional manner. - Install, configure, and maintain desktops, laptops, and other end-user devices. - Manage user accounts, including creation, modification, and password resets across various systems. - Escalate complex and unresolved issues to the appropriate senior IT personnel. - Meticulously document all support incidents, from initial contact to final resolution, within the help desk system. - Assist in creating and maintaining IT documentation, user guides, and knowledge base articles. - Provide basic training and guidance to staff on the use of hardware and software to promote efficiency and best practices. Qualifications and Skills - Proven experience working in an IT help desk or a similar technical support role. - Strong working knowledge of Windows operating systems and Microsoft 365/Office Suite. - Excellent problem-solving and analytical abilities with a keen attention to detail. - Exceptional communication and interpersonal skills, with the ability to explain technical concepts clearly to non-technical users. - A strong customer service orientation and a patient, positive attitude. - Excellent organisational and time-management skills, with the ability to prioritise tasks effectively. - A recognised IT certification (such as CompTIA A+ or Network+) would be a significant advantage. - Basic understanding of networking principles (TCP/IP, DNS, DHCP) is desirable.
