</li></ul>On an An Average Day you Will:
(Includes, but not limited to)
<ul><li>Complete an initial, comprehensive and ongoing comprehensive assessment of patient and family to determine hospice needs</li><li>
Provide a complete physical assessment and history of current and previous illness(es)</li><li>
Provide professional nursing care by utilizing all elements of the nursing process</li><li>
Assess and evaluate patient&rsquo;s status by:</li><li>
Writing and initiating plan of care</li><li>
Regularly re-evaluating patient and family/caregiver needs</li><li>
Participating in revising the plan of care, as necessary</li><li>
Initiate the plan of care and make necessary revisions as patient status and needs change</li><li>
Use health assessment data to determine nursing diagnosis</li><li>
Develop a care plan that establishes goals, based on nursing diagnosis
</li><li>Include the patient and the family in the planning process</li><li>
Record pain/symptom management and changes/outcomes as appropriate</li><li>
Communicate with the physician regarding the patient&rsquo;s needs and report changes in the patient&rsquo;s condition; obtain/receive physicians&rsquo; orders, as required</li><li>
Teach the patient and family/caregiver self-care techniques, as appropriate</li><li>
Work in concert with the interdisciplinary group</li><li>
All other duties as assigned
</li></ul>Requirements:
<ul><li>Must have at least one (1) year of acute care experience in an institutional setting</li><li>
One (1) year of experience in home care preferred</li><li>
Must have a current licensure in the state</li><li>
Must understand the philosophy of hospice concept and understand needs of the terminally ill</li><li>
Must be flexible in work hours</li><li>
Must demonstrate nursing skills per competency checklist</li><li>
Must demonstrate a willingness to maintain comprehensive working knowledge regarding information systems and applicable software programs</li><li>
Ability to travel throughout the Bristol Hospice Service Area
