As a Care Consultant (Hospice Liaison), you will take a consultative, community-focused approach to building and strengthening relationships with referral sources. Your efforts will help connect patients and families with the exceptional end-of-life care they deserve.

Travel: You will be required to use your own vehicle to travel throughout the Bristol Hospice service area (mileage reimbursement applies).

On an Average Day You Will:
 
(includes not limited to)
<ul><li>Build relationships in assisted living facilities, hospitals, physician offices, personal care homes, and other community referral sources</li><li>
Be out in the community (boots on the ground) every day, serving as the face of Bristol Hospice</li><li>
Make outreach calls and deliver impactful presentations to current and prospective referral sources</li><li>
Collaborate closely with our clinical and operational teams to ensure seamless care and promote our commitment to clinical excellence</li><li>
Ensure our patients and their families receive the appropriate information about hospice care</li><li>
Meet with community partners regarding hospice services for their patients</li><li>
Participate in regular collaborative team calls</li><li>
Develop and maintain relationships of trust and integrity within the community
</li><li>Drive continual growth of Bristol Hospice through advocacy with referral sources
</li></ul>Ideal Candidate Meets the Following Requirements:
<ul><li>Ability to communicate with general public and represent Bristol in a positive manner</li><li>
Ability to articulate the Hospice Conditions of Participation and facilitate the transitions in care for hospice patients, family members, caregivers and other members of the healthcare continuum
</li><li>Ability to create relationships with referral sources, patients, and families
</li><li>Minimum of (1) year of sales, marketing, professional relations and/or education training required
<ul><li>Experience in one of the following settings has prepared you for this role: hospice sales, home health sales, medical device sales, clinical liaison, RN liaison, clinical coordination or medical sales</li></ul></li><li>
Flexible with scheduling including some evenings and weekends to ensure patients and families needs for hospice services can be met

On an Average Day You Will:
 
(includes, but not limited to)
<ul><li>Provide medical care, palliate and manage terminal illness in compliance with the state Nurse Practice Act under the supervision of a collaborating physician to adults and children</li><li>
Complete history and physical examinations by evaluating the patient's medical condition and health history, ordering and interpreting results from laboratory and diagnostic tests or procedures, diagnosing health conditions and documents the findings in the patient's medical record</li><li>
Manage health conditions and terminal illness by developing an individualized plan of care, prescribing medications and/or treatments, obtaining consultations or making referrals and coordinating hospice care services</li><li>
Complete the hospice face to face visit requirements prior to the third and subsequent hospice benefit periods to assess hospice eligibility based on the hospice conditions of participation and the local coverage determinations</li><li>
Provide clinical information to the hospice medical director for interpretation of eligibility for hospice recertification</li><li>
Make recommendations to the hospice medical director regarding patient eligibility for certification and recertification for hospice care</li><li>
Review clinical information, coordinate and oversee patient care in consultation with the hospice interdisciplinary team and other healthcare practitioners</li><li>
Assure that patient receives appropriate measures to control symptoms, through collaboration with interdisciplinary team members</li><li>
Review and develop protocols for treatment and propose options for interventions based on the literature in collaboration with the collaborating physician</li><li>
Consult the collaborating physician when the patient's plan of care is outside standardized practice and protocols</li><li>
Collaborate with the interdisciplinary group to create, review and revise the patient's plan of care</li><li>
Fulfill the obligation of requested and/or accepted assignments</li><li>
Demonstrate knowledge in communication and counseling patient/family in dealing with end-of-life issues</li><li>
Complete all other duties as assigned
</li></ul>Requirements:
<ul><li>Must be a graduate of an accredited School of Nursing and accredited Nurse Practitioner Program</li><li>
Must have current nursing licensure in State</li><li>
Must have a master's degree with a minimum of one (1) year Nurse Practitioner experience and a minimum of one (1) year hospice or palliative care experience</li><li>
Must have accreditation from an approved certifying body for advanced practice nursing as required by the State</li><li>
Certification in a specialist area preferred; (e.g. Hospice and Palliative Nursing (CHPN), Pediatrics or Geriatrics)</li><li>
Must be a licensed driver with licensed automobile that is insured in accordance with 
organization/state requirements and is in good working order</li></ul>
