GENERAL SUMMARY: 
Responsible for serving on the patient care team for an assigned area including direct patient care as an unlicensed professional and under the direct supervision of a Registered Nurse (RN). Incorporates the primary job functions of a certified nursing assistant (CNA) and other duties related to the care of patients, dependent upon the work area and/or patient populations to which they are assigned. Reports to Nurse Manager, Assistant Nurse Manager, Charge Nurse, Administrative Manager or Staff RN. Employees holding nursing licensure will be held accountable to Nebraska nursing licensure requirements, and must maintain licensure or an active Nebraska CNA.
 
PRINCIPAL JOB FUNCTIONS: 
1. *Commits to the mission, vision, beliefs and consistently demonstrates our core values.
 
2. *Recognizes and reacts to emergency procedures per role and policies/procedures.
 
3. *Answers telephones and assigned communication devices as appropriate and relays information as necessary. Refers all physician telephone orders to a licensed professional.
 
4. Assists in collecting specific information from patients and contributes to admission, transfer and dismissal processes when delegated by a RN, and with responsibility for patient assessment being performed by a RN; assures timely reporting and recording of patient's Electronic Medical Record (EMR) .
 
5. *Assists with patient care that is consistent with standards of care and clinical priorities determined by the RN with the nature of the area/unit, patient care division policies and procedures, demonstrated competencies, and with consideration given to risk management and infection control practices.
 
6. *Performs and documents routine direct nursing care tasks according to identified age-specific needs.
 
7. Obtains and records vitals, height, weight, and bedside glucose results; reports results to appropriate personnel.
 
8. *Performs Activities of Daily Living (ADLs) for assigned patients and documents in the EMR.
 
9. *Responsible for labeling and securing patient care belongings and ensuring that the patient room is safe and uncluttered.
 
10. *Distributes patient’s meals/water and assists with feeding of patients as needed.
 
11. Assists with ambulation, lifting and positioning according to procedure, including the appropriate use of patient lift devices and use of good body mechanics.
 
12. Assists with transporting patients, including at time of transfer and/or dismissal.
 
13. *Assists patients with elimination needs and measures/records bodily elimination in medical record.
 
14. Bathes patients as directed by the RN.
 
15. Participates as a member of the team in Hourly Patient Rounds to proactively address patient needs.
 
16. Provides close observation or one-to-one care, as directed by the RN, to prevent patients from wandering or potential injury.
 
17. Collects, accurately labels and transports specimens to lab as required.
 
18. Assists with dressing changes, applying splints, setting up suturing and procedures, and wound care on closed wounds as directed by RN.
 
19. *Cleans, stocks and cares for supplies and equipment in assigned work area.
 
20. Performs other related projects and duties as assigned.

REQUIRED KNOWLEDGE, SKILLS AND ABILITIES: 
1. Maintains clinical competency as required for the unit of assignment including but not limited to population-specific competencies and participates in annual skill competency verification.
 
2. Knowledge of general hospital processes and procedures.
 
3. Knowledge of computer equipment and software applications relevant to work functions.
 
4. Ability to communicate effectively both verbally and in writing.
 
5. Ability to work in a fast-paced environment related to changing patient needs, including work with patients with acute, chronic and complex disease processes, and those who are dying.
 
6. Ability to establish and maintain effective working relationships with health care team members and diverse populations.
 
7. Ability to maintain confidentiality of patient information.
 
8. Ability to practice proper body mechanics to ensure personal and customer/patient safety.
 
9. Ability to prioritize work demands and work with minimal supervision.
 
10. Ability to maintain regular and punctual attendance.
 
EDUCATION AND EXPERIENCE: 
Active Nebraska Certified Nursing Assistant (CNA) credential required. Six (6) months CNA experience preferred.
 
OTHER CREDENTIALS / CERTIFICATIONS: 
Basic Life Support (CPR) certification required. Bryan Health recognizes American Heart Association (for healthcare professionals), American Red Cross (for healthcare professionals) and the Military Training Network.
 
If assigned to the Emergency Department, may have an EMT certification in place of CNA.
 
PHYSICAL REQUIREMENTS: 
(Physical Requirements are based on federal criteria and assigned by Human Resources upon review of the Principal Job Functions.)
 
(DOT) – Characterized as medium work requiring exertion of 20 to 50 pounds of force occasionally, and/or 10 to 25 pounds of force frequently, and/or a greater than negligible up to 10 pounds of force constantly to move objects.

Patient Care Technician-Functional Care

<div><div>SUMMARY:
The Department of Pharmacy has the responsibility for the implementation and coordination of drug control in the process of providing pharmaceutical care for patients of all ages within the Medical Center. The Clinical Pharmacist has the direct responsibility for the provision of quality and timely care to the patients of the Medical Center.
RESPONSIBILITIES INCLUDE BUT ARE NOT LIMITED TO:

</li></ul><ul><li>Utilizes and maintains patient profiles; monitors drug therapy for efficacy, drug interactions, adverse drug reactions, contraindications, allergies, sensitivities, and cost; practices in a proactive, self-directed manner.
</li></ul><ul><li>Ensures that medications and combinations of medications dispensed constitute rational drug therapy; if questions arise regarding the rational nature of drug therapy, the Staff Pharmacist shall contact the appropriate health professional(s) in the course of patient care.
</li></ul><ul><li>Participates in Pharmacy Department and Medical Center programs which evaluate the use of pharmaceuticals in the course of patient care.
</li></ul><ul><li>Encourages prescribers and other health professionals to use the Medical Center formulary; maintains control of the formulary system by contacting prescribers and other health professionals when a non-formulary drug is ordered and suggests appropriate formulary alternatives.
</li></ul><ul><li>Performs discharge medication teaching for patients receiving discharge medications from the Medical Center pharmacies and/or upon request.
</li></ul><ul><li>Performs pharmacokinetic dosage calculations and evaluations as determined by the therapeutic needs of the patient.
</li></ul><ul><li>Participates in Pharmacy Department and Medical Center quality assessment and improvement programs as requested.
</li></ul><ul><li>Assists in the orientation, training, and assessment of Department personnel.
</li></ul><ul><li>Participates in Pharmacy Department assisted investigational drug studies to ensure appropriate dispensing and required maintenance of research records.
</li></ul><ul><li>Is accountable for the timely, safe, and accurate distribution of medications, including the frequent use of professional judgment with regard to medication acquisition, storage, packaging, distribution, and use.
</li></ul><ul><li>Is accountable for the transcription and evaluation of physician medication orders, preparation of patient specific unit dose medications, preparation of sterile admixture solutions, extemporaneous compounding, controlled substance distribution, and outpatient medication preparation / dispensing.
</li></ul><ul><li>Ensures the quality of pharmaceuticals within the Medical Center by performing assigned patient care area inspections and removing from the assigned areas recalled, defective, expired, or inappropriately stored pharmaceuticals.
</li></ul><ul><li>Clinically supervises pharmacy technicians throughout their shift to ensure the completion of assigned tasks; responsible for the tasks completed by the pharmacy technician.
</li></ul><ul><li>Adheres to safety and security procedures to promote a safe and secure environment.
</li></ul><ul><li>Compounds and dispenses medication orders accurately using proper labeling and documentation according to Pharmacy policies and procedures, State law and Federal law.
</li></ul><ul><li>Uses available reference sources to respond to drug information requests and as appropriate refer questions to health professionals with a more extensive experience with the particular request.
</li></ul><ul><li>Reports and documents medication variances.
</li></ul><ul><li>Provides accurate drug information regarding drug therapy indications, dosage, administration, adverse effects, interactions, IV compatibility and formulary recommendations to physicians, nurses, patients, and
</li></ul><ul><li>Prioritizes workload and clinical interventions and assists Department and Medical Center staff as needed to ensure that optimal patient care is provided.
</li></ul><ul><li>Participates in Pharmacy Department education and training programs as requested.
</li></ul><ul><li>Attends all mandatory Medical Center inservices and meetings; attends Department staff meetings as schedules permit.
</li></ul>QUALIFICATIONS:
Entry-level degree (B.S., Pharm.D.) from a college of pharmacy required. Completion of a hospital pharmacy residency accredited by the American Society of Health-System Pharmacists preferred. Pharmacist licensure in the State of Nebraska required.

Clinical Pharmacist - Internal Medicine/Central

Performs general equipment cleaning and stocking. Prepares machines and other equipment for sonographer use. Performs diagnostic electrocardiograms as well as cardiac monitor placement. Cleans and processes transesophageal probes.
 
PRINCIPAL JOB FUNCTIONS: 
1. *Commits to the mission, vision, beliefs and consistently demonstrates our core values.
 
2. *Participates in identifying and reporting patient safety issues.
 
3. *Assists cardiovascular sonographers and coordinators as requested.
 
4. *Cleans, disinfects and sterilizes all cardiac and vascular equipment (ultrasound machines, ultrasound probes including transesophageal probes, ECG equipment, etc.), following current infection control guidelines.
 
5. *Performs diagnostic electrocardiograms, signal-averaged electrocardiograms, flight physical electrocardiograms, and holter and event monitor applications in accordance with established departmental policies.
 
6. *Takes calls from account hospitals for "Stat" overreads; contacts Cardiologist 24/7 for urgent interpretation.
 
7. *Receives event monitor transmissions of ECG strips and contacts Cardiologist for "Stat" overreads.
 
8. *Recognizes and reports "Discrepancy ECG".
 
9. *Prepares unconfirmed ECG's for cardiologist overread.
 
10. *Transcribes cardiologist overreads, creating a confirmed ECG; distributes copies of confirmed ECG.
 
11. *Performs routine equipment maintenance.
 
12. *Transports patient and/or equipment, alone or with assistance, in bed, wheeled cart or wheel chair to designated area.
 
13. Informs supervisor of any institution or equipment concerns.
 
14. Maintains a safe working environment.
 
15. May deliver medications received on unit to the appropriate patient, RN, physician, medication drawer or medication storage place.
 
16. Maintains professional growth and development through seminars, workshops, and professional affiliations to keep abreast of latest trends in field of expertise.
 
17. Participates in meetings, committees and department projects as assigned.
 
EDUCATION AND EXPERIENCE: 
High school diploma or equivalency required. Current progress towards a cardiovascular sonography degree required. Courses related to cardiovascular field at post-secondary level required including ECG course. Basic Life Support (CPR) with AED certification within three (3) months of hire required. Basic Life Support (CPR) certification required.
