What you'll get to do as a Community Support Specialist:<ul><li>
Provide support and activities to clients through an organized treatment plan designed to ease an individual's immediate and continued adjustment to community living. You'll be working in the community which will include clients homes, clinics, and other places around the community as part of your regular assigned case load.</li><li>
Utilize a supportive, educational approach to promote improved functioning to meet the needs of the client, the goals of treatment while encouraging recovery and increased resiliency.</li><li>
Provide and/or coordinate a comprehensive follow-up to all clients for whom he/she has been responsible, including follow up for missed appointments, and conducting outreach and engagement as needed.</li><li>
Use advanced knowledge of psychological concepts, evidenced based interventions and coordinating delivery of mental health services with other services provided by practitioners and community partners.</li><li>
Use specialized knowledge of behavioral science principles such as: Motivational interviewing, Reinforcement, Operant conditioning, Social norming, Attitude change, Persuasion, Hierarchy of needs, Interpersonal and group dynamics, Cognitive processing, Attentional processes, Personality and empirically supported behavioral intervention strategies.</li></ul>

In this role, you will lead essential initiatives that strengthen clinical quality, optimize provider performance, and enhance the overall care delivery experience across the organization.
