Job Summary: 
The Community Support Supervisor provides support to a team of Community Support Specialists by using advanced knowledge of psychological concepts, evidenced-based interventions, and coordinating delivery of mental health services to lead their team in conjunction with other services provided by practitioners and community partners. This role is vital in ensuring that our team members have the guidance and resources they need to provide effective support to the youth in our community. In this position, the supervisor conducts initial and annual psychosocial assessments and develops client-centered, strengths-based treatment plans. The role involves regular interactions with the community and requires a strong commitment to teamwork and leadership.

Key Responsibilities:<ul><li>
Hold regularly scheduled individual supervisions and administrative team meetings.</li><li>
Provide clinical oversight and guidance for client treatment on an as-needed basis, including appropriate addendums to annual treatment plans.</li><li>
Assist team members in identifying case management needs.</li><li>
Review all critical intervention plans, engaging in direct crisis intervention during regular business hours and as needed after hours or on weekends.</li><li>
Complete all documentation of services in a timely manner as outlined by the P&amp;Ps.</li><li>
Regularly review and monitor written documentation to assure compliance with documentation requirements, particularly for new team members on probation.</li><li>
Provide coaching and growth opportunities for team members through feedback and praise.</li><li>
Complete intakes and annual assessments, including documenting diagnoses, psychosocial histories, key clinical findings, and recommendations.</li><li>
Facilitate the hiring and training of new staff as needed.</li><li>
Evaluate each team member according to performance evaluation guidelines.</li></ul>
Education, Experience, and/or Credential Qualifications:<ul><li>
Master&rsquo;s degree in Psychology, Sociology, Social Work, or related field &ndash; Preferred</li><li>
Must have or obtain a valid Driver&rsquo;s License within two (2) weeks of employment</li><li>
Must have access to a personal vehicle for occasional transport of clients.</li><li>
Proficient in Microsoft Office applications</li></ul>

Must be able to establish effective relationships and demonstrate exceptional customer service skills via telephone, email, and in-person interactions.</li><li>
Must be able to follow direction and accept supervision.</li><li>
Must have excellent knowledge of community resources and the ability to engage community partners effectively in the treatment of youth.</li><li>

Licensed Crisis Therapist (Saturday & Sunday 8am-6pm)

<div>Job Description:
The Registered Nurse (RN) position is responsible for providing nursing services to clients/patients in a variety of settings to include but not limited to: residential care facilities, behavioral health programs, substance use disorder treatment programs, crisis centers, outpatient and/or primary medical care clinics or in the community within client/patient homes.
Essential Job Functions:<ul><li>
Perform all appropriate nursing assessments per service line requirements, utilizing a high level of clinical competence.</li><li>
Evaluate the client/patient&rsquo;s physical condition and the need for services.</li><li>
Complete required screenings, record the client/patient&rsquo;s initial medical histories and vital signs; coordinate any required action planning based on results.</li><li>
Monitor the health status of the client/patient during services provided.</li><li>
Work with the treatment team to coordinate care based on client/patient needs.</li><li>
Create action plans for client/patient based on physical and mental health in regards to treatment needs.</li><li>
Oversee infectious disease control, client/patient hygiene, and related client &amp; facility health issues, under the supervision of nursing leadership.</li><li>
Provide disease prevention, risk reduction and other health education as needed.</li><li>
Arrange medical appointments for client/patients dealing with medical conditions that occur during treatment and manage medical emergencies, such as accidental injuries and/or drug reactions.</li><li>
Consult with a physician/advanced practice provider as necessary or advocate for medical services through managed care organizations.</li><li>
Arrange or monitor special dietary needs for medical conditions.</li><li>
Review medication requirements with clients/patients, educating the client and guardian (if necessary) about the benefits of taking prescribed medications as prescribed.</li><li>
Administer and oversee administration of medications via various methods according to service line requirements; Monitor medication compliance in regard to State Regulatory standards.</li><li>
Consult with the physician/advanced practice provider or pharmacy to confirm medications prescribed.</li><li>
Oversee therapeutic injection of medication (subcutaneous or intramuscular).</li><li>
Monitor lab levels including consultation with physicians/advanced practice providers, clients, and clinical staff.</li><li>
Coordinate medication needs with pharmacies, clients/patients, and families, including the use of indigent drug programs.</li><li>
Monitor medication side-effects including the use of standardized evaluations.</li><li>
Monitor physician orders for treatment modifications requiring client/patient education.</li><li>
Assists Physician/Advanced Practice Provider with client/patient exams.</li><li>
Sets up and draws any necessary labs and performs any special procedures as requested by the Physician/Advanced Practice Provider.</li><li>
Complete all required documentation in a timely manner as indicated in company policies and procedures.</li><li>
Demonstrate interest in long term and short term goals and objectives of the company.</li><li>
Other duties as assigned by program leadership.</li></ul>

Detailed knowledge of information and techniques needed to rehabilitate physical and mental ailments and provide alternative treatments.</li><li>
Aptitudes to develop, coordinate, and evaluate services within assigned program responsibilities.</li><li>
Effectively apply techniques of a complex nature to make a decision or solve a problem by using logic to identify key facts, explore alternatives, and propose quality Solutions.</li><li>
Ability to read &amp; interpret documents such as lab results, notes &amp; documents within an electronic health record, safety rules, operating &amp; maintenance instructions, and procedure manuals.</li><li>
Ability to write routine reports &amp; correspondence clearly &amp; effectively via multiple methods, in various platforms. Communicates effectively &amp; therapeutically to clients, family members, employees, &amp; other contacts within the community in-person, via telephone, and via video conferencing.</li></ul>
Experience and Education Qualifications:<ul><li>
Current Registered Nurse licensed in the state of practice<ul><li>
Must be willing to become licensed in additional states, other than the state currently licensed in. Must be willing and comfortable with providing telehealth services to the individuals we serve.</li></ul></li><li>
FQHC: BCLS certification required, ACLS certification preferred.</li></ul>
