Key Responsibilities:<ul><li>
Collaborate with the ERE Coordinator to serve as the first point of contact for ERE referrals from Behavioral Health Response and the system&rsquo;s sobering center.</li><li>
Conduct community outreach to engage consumers referred to ERE and perform initial assessments of their needs.</li><li>
Establish rapport with consumers, facilitating discussions about their strengths, needs, and preferences to motivate recovery.</li><li>
Provide crisis intervention and perform preliminary linkage and coordination of services for individuals seeking treatment.</li><li>
Offer short-term case management and maintain regular contact with consumers and service providers to monitor service delivery.</li><li>
Report on individual outcomes at 30-day, 3-month, and 6-month intervals.</li><li>
Maintain accurate electronic records and documentation as required by the Department of Mental Health.</li><li>
Collaborate with the ERE Outreach Team and participate in regular meetings and training sessions.</li><li>
Build and maintain relationships with hospital staff and community resources to ensure appropriate referral patterns.</li></ul>
Education, Experience, and/or Credential Qualifications:<ul><li>
Be at least 21 years of age.</li><li>
Minimum of six months related experience in the Behavioral Health Field, including case management and outreach.</li><li>
Graduate from an accredited college or university with a Bachelor&rsquo;s Degree in social work, counseling, psychology, or a closely related field.

At least two years of relevant, qualifying full-time equivalent experience in human service delivery, demonstrating skills in developing positive client relationships and navigating complex service systems. 
OR</li><li>
Possess the Certified Peer Specialist credential.</li></ul>
Additional Qualifications:<ul><li>
Successful completion of background checks, including criminal record, driving record, abuse/neglect, and fingerprint checks.</li><li>

Child and Adolescent Psychiatrist (Hybrid)

<div>Job Description:
The Registered Nurse (RN) position is responsible for providing nursing services to clients/patients in a variety of settings to include but not limited to: residential care facilities, behavioral health programs, substance use disorder treatment programs, crisis centers, outpatient and/or primary medical care clinics or in the community within client/patient homes.

Perform all appropriate nursing assessments per service line requirements, utilizing a high level of clinical competence.</li><li>
Evaluate the client/patient&rsquo;s physical condition and the need for services.</li><li>
Complete required screenings, record the client/patient&rsquo;s initial medical histories and vital signs; coordinate any required action planning based on results.</li><li>
Monitor the health status of the client/patient during services provided.</li><li>
Work with the treatment team to coordinate care based on client/patient needs.</li><li>
Create action plans for client/patient based on physical and mental health in regards to treatment needs.</li><li>
Oversee infectious disease control, client/patient hygiene, and related client &amp; facility health issues, under the supervision of nursing leadership.</li><li>
Provide disease prevention, risk reduction and other health education as needed.</li><li>
Arrange medical appointments for client/patients dealing with medical conditions that occur during treatment and manage medical emergencies, such as accidental injuries and/or drug reactions.</li><li>
