JOB DESCRIPTION:
SUMMARY OF POSITION
Responsible for development, staffing, marketing, organization, and management of services. Provides clinical staff and interns/practicum students with clinical supervision and/or direct supervision. Works closely with the care teams to ensure regulatory compliance. Provides counseling services. Provides clinical supervision. Conducts intake and assists with making linkages to internal and external resources. Provides and promotes outstanding internal and external customer service.
ESSENTIAL DUTIES &amp; RESPONSIBILITIES<ul><li>
Provides counseling services using evidence-based methodologies to provide excellent clinical services. Prepare for and conduct individual, group, and family therapy for assigned caseload. Works with assigned staff to help them develop competencies in same.</li><li>
Provides clinical supervision and consultation to substance use clinicians and interns/practicum students, as assigned to assure quality of services. Trains or ensures training of new staff/Interns/practicum students in various aspects of services to comply with regulatory guidance.</li><li>
Ensures that substance use clinical services are recovery focused taking into account trauma informed care practices. Provides consistent, personal growth-focused services to all assigned clients by maximizing time spent working directly with clients to assist their personal growth process.</li><li>
Requires a high level of collaboration and communication across multiple levels of management. Collaborates across nursing, substance abuse outpatient treatment, residential treatment programs to transfer clients among various levels of care, and educate staff members about services.</li><li>
Ensures specialty annual requirements are met by assigned staff (i.e., TB testing, Hepatitis testing, Narcan training, etc.)</li><li>
Conducts intake for recipient entering services, including explaining services, gathering information needed on intake materials, ensuring service recipient understands all forms, assuring signature on appropriate contracts and consents, and submitting materials for processing, as assigned. Conducts assessment and write assessment summaries, as assigned.</li><li>
Assists service recipients with making linkage with internal and external resources as outlined in treatment plan and in coordination with health home, and other services as appropriate. Maintains appropriate contact with referral sources including providing information on service recipient progress, behavior, and needs (with appropriate releases), and maintaining ongoing consultation when appropriate.</li><li>
Assists service recipient in making discharge plans and provide follow up services as assigned.</li><li>
Monitors urinalysis, breathalyzer, and other testing as assigned.</li><li>
In the absence of supervisor, in consultation with other leaders, makes decisions to assure program continuity of services.</li></ul><div>

JOB DESCRIPTION:
SUMMARY OF POSITION 
This position provides in home visitation services to enrolled participants, providing linkage and referral to other services and support to enhance family functioning by: building trusting relationships, teaching problem-solving skills, and improving the family&rsquo;s support system.
LOCATION &amp; TRAVEL REQUIREMENTS<ul><li>
This position provides in-home services throughout Putnam County and requires regular travel within the county.</li><li>
The primary service area is Cookeville, TN, with work performed throughout surrounding communities in Putnam County.</li><li>
Occasional evening and weekend community events may be required to support outreach and program initiatives.</li><li>
Attendance at the annual Tennessee State Summit is required. The summit location is determined by the funding agency and will be held within the state of Tennessee.</li></ul>
COMPENSATION
Compensation begins at $17.00 per hour. Candidates with relevant professional experience beyond the bachelor's degree may be considered for a higher starting rate based on qualifications and directly related experience.
ESSENTIAL DUTIES &amp; RESPONSIBILITIES 
Responsible for initiating and maintaining regular and long-term (up to five years) contact and support with families. This activity will occur primarily within the family&rsquo;s home; with visits lasting around an hour. 
The interventions should be family-centered, strength-based, and directed at establishing a trusting relationship; strengthening the parent-child relationship; promoting healthy childhood growth and development; and enhancing family well-being by reducing risk and building protective factors. 
Partners with families and honors diverse family structures and parenting practices. 
Activities may also include but are not limited to: administering the Family Resilience and Opportunities for Growth Scale (FROG) Scale; administering screening tools such as the ASQ, ASQ-SE, Depression Screens etc.; identifying and referring families for other supportive services, including health care services. 
Responsible for assisting the family in establishing goals and supporting them throughout this process and is responsible for implementing activities outlined on the Family Service Plan to address areas of concern that were identified on the FROG scale. 
Assist in preparation of required monthly, quarterly and annual statistical reports. 
Maintains up-to-date and accurate records for families on GGK curriculum, HFA data, and other required records. 
Participate in training according to requirements of HFA guidelines, GKI and DOH contract. 
Implements GGK curriculum and HFA strategies to enrollees through weekly home visits. 
Provides case management to families on caseload. 
Attend training and other mandatory meetings.

SUMMARY OF POSITION
Functions as staff LPN utilizing the nursing process with performing comprehensive psychiatric /mental health assessments. Evaluates mental status and refers clients to other treatment providers when necessary. Assumes primary responsibility and accountability for ones nursing practices as defined in the state specific Code of Nursing Practice, as it pertains to a licensed practical nurse and functions under the supervision of an RN or physician.
he Columbia clinic is seeking a team-oriented LPN to join a team of therapists, nurse practitioners, psychiatrists, case managers, and support staff to deliver care that saves lives. Mental health experiences preferred. At least two years' experience in the field as an LPN. This is an office-based job. Hours of Monday through Friday 8-5 and is not telehealth or is not a work from home position.
ESSENTIAL DUTIES &amp; RESPONSIBILITIES<ul><li>
Performs comprehensive psychiatric /mental health assessments, evaluates mental status and refers to other treatment providers when necessary.</li><li>
Monitors changes in health status, including but not limited to vital signs, weights, nutritional status and disease states.

The Centerstone Contact Center Specialist is an integral component of our Centerstone team. This position often serves as the first point of contact our patients have with Centerstone. The position requires excellence in customer service and compassionate care supported by a thorough understanding of the organization&rsquo;s treatment and support programs. The ideal candidate must demonstrate a working knowledge of behavioral health basics and understand health insurance benefits.
Compensation: The entry pay range for this position is $16.50 to $18.22 per hour, with the final starting rate determined based on relevant experience and qualifications.

JOB DESCRIPTION:
Position Summary:
The Director, Adult Community Services provides strategic, operational, and financial leadership for adult community-based behavioral health services across Centerstone&rsquo;s Florida market. This position oversees assigned service lines through program managers and is responsible for program performance, financial stewardship, compliance, community partnerships, and the development and expansion of adult community services.
Essential Duties and Responsibilities:
<ul><li>Leads through managers to support accountability, workforce development, service quality, and alignment with organizational priorities.</li><li>
Provides strategic and operational oversight for adult community-based behavioral health services across Centerstone&rsquo;s Florida market.</li><li>
Champions change and reinforces a collaborative, team-based approach across programs and departments</li><li>
Leads budget development, financial oversight, billing integrity, revenue cycle performance, and fiscal accountability for assigned programs.</li><li>
Monitors key performance indicators, quality outcomes, operational data, and financial metrics to support decision-making and continuous improvement.</li><li>
Manages existing contracts, grants, and service agreements and ensures compliance with regulatory, payer, accreditation, certification, fidelity, documentation, and agency standards, including audit readiness and corrective action planning.</li><li>
Builds and maintains strong relationships with community partners, healthcare systems, referral sources, funders, coalitions, and other stakeholders.</li><li>
Represents adult community services in internal committees, interdepartmental workgroups, public forums, and external meetings as appropriate.</li><li>
Supports the development, expansion, and redesign of adult community services in response to organizational priorities, community need, and funding opportunities.</li><li>
Participates in required meetings, staff development activities, and special projects as assigned.</li></ul>
Supervisory Requirements: This position has supervisory responsibilities, which may include providing direction, training, coaching, and performance feedback in accordance with system policies.
Knowledge, Skills, and Abilities:<ul><li>
Strong leadership and management skills with the ability to lead multiple adult community-based service lines through managers.</li><li>
Demonstrated ability to oversee program performance, budgeting, contracts, billing, revenue cycle operations, and key performance indicators.</li><li>
Strong knowledge of adult behavioral health systems, recovery-oriented care, intensive community support models, substance use services, and prevention and outreach programming.</li><li>
Ability to analyze operational, financial, and clinical data to support decision-making, performance improvement, and strategic planning.</li><li>
Strong understanding of regulatory, accreditation, payer, and documentation requirements applicable to adult community-based services.</li><li>
Excellent communication, relationship-building, and presentation skills with internal and external stakeholders.</li><li>
Ability to manage competing priorities, lead change, and maintain confidentiality in accordance with HIPAA and organizational policy.</li></ul>
Minimum Qualifications:
Education:<ul><li>
Master&rsquo;s degree in Social Work, Counseling, Psychology, Human Services, Business Administration, Healthcare Administration, or a related field from an accredited institution</li><li>
A combination of education, training, and relevant experience may be considered in lieu of the stated minimum education, where permitted by regulation.</li></ul>
Experience:<ul><li>
7 years of progressively responsible experience in adult behavioral health, substance use, community services, or related program operations.</li><li>
5 years of supervisory or management experience, preferably including oversight of multiple programs, sites, service lines, or managers.</li><li>
Demonstrated experience with operational oversight, including program performance, contracts, budgets, regulatory compliance, billing, and service delivery outcomes.</li><li>
Knowledge of adult community behavioral health systems, recovery-oriented services, intensive community support models, and community-based service coordination.</li><li>
Experience working with payer, documentation, utilization, and performance requirements in publicly funded or managed care environments.</li></ul>
Licenses and Certifications:<ul><li>
Valid driver&rsquo;s license, acceptable driving record, and ability to travel between service locations as required.</li></ul>
Preferred Qualifications:
<ul><li>Clinical licensure in the Florida, such as LCSW, LMHC, LMFT, or equivalent.</li><li>
Experience overseeing multiple adult community-based behavioral health programs or service lines.</li><li>
Experience with FACT teams, targeted case management, community-based mental health and substance use programming, and prevention and outreach services.</li><li>
Experience with program development, service expansion, grant implementation, or contract start-up.</li><li>
Demonstrated success using data, dashboards, and key performance indicators to improve operational performance, compliance, financial performance, and service outcomes.</li><li>
Experience collaborating with managed care entities, funders, healthcare systems, and community partners.</li><li>
Knowledge of recovery-oriented, trauma-informed, and person-centered service delivery models.
</li></ul>Physical and Work Environment Requirements:
Work is performed in an office, clinical, residential, and/or community-based environment depending on role and assignment. SUMMARY OF POSITION
Provide clinical services consistent with established guidelines for best practice interventions, including evaluation, individual, group, family, or couple&rsquo;s psychotherapy for adults, adolescents and children seen in one&rsquo;s service area.
