JOB DESCRIPTION:
SUMMARY OF POSITION 
Work as a member of a collaborative team to provide screening, assessment and treatment (including individual, group &amp; family) to treat children with mental health or co-occurring issues. Services will be provided in the Manatee schools. Formulate and implement solution-focused treatment plans. Work cooperatively with community agencies to implement services and link clients to available programs and community services. Collaborate with CPS and child welfare programs.
ESSENTIAL DUTIES &amp; RESPONSIBILITIES<ul><li>
Develop therapeutic rapport with clients &ndash; Meet with clients as scheduled to aid in development of therapeutic relationships.</li><li>
Diagnostic Assessment &ndash; Formulate a provisional diagnostic assessment based on information presented, identify need for risk assessment.</li><li>
Concurrent Documentation &ndash; Completes documentation within 24 hours. Provide individual, family and group therapy to individuals for mental health and/or substance abuse disorders.</li><li>
Screenings &ndash; Complete screenings for depression, suicide, drug use, home assessment, etc.</li><li>
Treatment Planning &ndash; Developing, writing, implementing, evaluating and revising overall treatment goals/objectives and plans per ongoing assessment of needs and input from the individual using individual strengths.</li><li>
Therapeutic Interventions &ndash; Use of empirically based modalities to engage and manage their mental health or substance abuse diagnosis.</li><li>
Referrals &ndash; Complete, document and follow up on referrals.</li><li>
Discharge/close referrals &ndash; Discharge clients who successfully complete therapy services and those who fail to follow through.</li><li>
Other Related Duties &ndash; Peer Reviews, consultation, attend staffing, work with community partners, state reporting, follow contract policies and processes, state reporting, etc.</li></ul>

This position is part of a 24/7/365 high volume, suicide prevention focused call team. Our department has been featured in a Time Magazine article detailing our team&rsquo;s efforts toward suicide prevention. As a part of this team, you&rsquo;d be responsible for providing brief clinical screenings, risk assessments, safety planning, crisis intervention, and appropriate service linkage for individuals who are experiencing an acute mental health crisis. SUMMARY OF POSITION
This case manager position will serve clients as part of Centerstone's Mobile Crisis team in Manatee County. This role will respond to crisis events in the local community and provide deescalation, wraparound support, and follow up services. This position will provide mental health, substance abuse and outreach services to individuals who may have a mental health and/or substance use disorder. This role will work with community partners and local stakeholders to provide comprehensive care. The Case Manager will meet with individuals in the office, hospital, and/or their own environment such as in the client&rsquo;s home, schools/work, etc. This role will ensure continuity of care with clients through case management services as well as link clients and their families to needed resources to help to improve their quality of life. A variety of clinical training opportunities are available based on program and interest.
ESSENTIAL DUTIES &amp; RESPONSIBILITIES<ul><li>
Reports to work as scheduled on a daily basis and performs duties as assigned by supervisor to ensure the smooth operation of the department.</li><li>
Provides individual and group mental health and substance use disorder case management services. Refers, advocates, and links client and family for additional services as identified.</li><li>
Provides supportive and educational counseling services (individual, family, and group) to develop daily living and coping skills. Assist clients with developing natural support systems.</li><li>
Transport clients in company vehicles to necessary appointments as needed.</li><li>
Responsible for developing, implementing, evaluating, and updating treatment plans with input from the client and family.</li><li>
Conduct clinical behavioral health assessments.</li><li>
Enters written documentation into Electronic Medical Record in a timely manner for all services rendered.</li><li>

Provides person-centered assistance to clients to help ensure appropriate continuum of care. Updates the care plan and monitors the effectiveness of client treatment. Coordinates treatment plan in support of increased self-sufficiency. Primary case holder for the client and is responsible for maintaining the chart to ensure the client has access to services provided by this program. Assesses and updates person-centered plan with the client and collaterals and monitors progress of the plan. Ensures client&rsquo;s clinical record is maintained and up to date at all times. Clinical documentation is accurate, meets organizations standards in composition and content, and is adequate for communication about client&rsquo;s treatment and progress. Assists in census management, admissions, and discharges. Communicates with staff and clients to help overall flow of facility.</li><li>
Completes authorizations paperwork for clients including ASAMs, Care Plans, CANS/ANSA&rsquo;s and all documentation/assessments necessary for treatment specific to the program.</li><li>
Meets with clients individually to explain insurance status and needs.</li><li>
Serves as the liaison between the client and the client&rsquo;s Representative Payee</li><li>
Uses structured, standardized interventions and resources for teaching to assist clients in overcoming barriers to recovery.</li><li>
Provides individual and/or group interventions in the home, community or office setting.</li><li>
Works within various community systems to collaborate on behalf of, and with, the client. Includes the criminal justice system, the child welfare system, hospital/medical systems, educational, etc.</li><li>

 Centerstone is hiring Case Managers to support a variety of mental health and substance use programs in Sarasota and Manatee counties. Positions are available across community-based and clinic-based teams, serving populations such as at-risk families, youth, and adults.
 
 
 Case Managers provide wraparound services, outreach, and care coordination to individuals who may have a mental health and/or substance use disorder. This role collaborates with community partners and local stakeholders to ensure comprehensive care. Services may be delivered in office settings, hospitals, or in the client&rsquo;s environment&mdash;including homes, schools, and workplaces.
 
 
 The primary duty as a Case Manager is to ensure continuity of care, while connecting clients and their families to resources that support improved quality of life.

 Essential Duties &amp; Responsibilities
 
 
 
 <ul>
 <li>
 Reports to work as scheduled on a daily basis and performs duties as assigned by supervisor to ensure the smooth operation of the department.
 </li>
 <li>
 Provides individual and group mental health and substance use disorder case management services. Refers, advocates, and links client and family for additional services as identified.
 </li>
 <li>
 Provides supportive and educational counseling services (individual, family, and group) to develop daily living and coping skills. Assist clients with developing natural support systems.
 </li>
 <li>
 Transport clients in company vehicles to necessary appointments as needed.
 </li>
 <li>
 Responsible for developing, implementing, evaluating, and updating treatment plans with input from the client and family.
 </li>
 <li>
 Conduct clinical behavioral health assessments.
 </li>
 <li>
 Enters written documentation into Electronic Medical Record in a timely manner for all services rendered.
 </li>
 <li>
 Act as liaison and communicate collaboratively with local law enforcement, and local stakeholders to maintain coordination of the treatment process.
 </li>

 Completes all required agency trainings to maintain 100% compliance.
 </li>
 <li>
 Maintains departmental productivity expectations.
 </li>
 <li>
 Additional training and/or certification may be required based on specific program guidelines.
 </li>
 <li>
 Completes all other duties as assigned.

 Knowledge, Skills &amp; Abilities
 
 
 
 <ul>

 Effectively communicate via written, verbal, in person and virtual methods.
 </li>
 <li>
 Knowledge and understanding of the characteristics of individuals with co-occurring issues, their families, and family systems.
 </li>
 <li>
 Good organizational and time management skills.
 </li>
 <li>
 Competency with electronic medical records, computer systems, and documentation.
 </li>
 <li>
 Strong interpersonal communication skills in order to collaborate with team members and maintain professionalism. Accountable and self-directed with daily job duties.
 </li>
 <li>
 Knowledge of community resources or ability to learn quickly as well as good organizational, time management and communication skills.
 </li>
 <li>
 Ability to develop supportive relationships with persons with mental health and substance use disorders while maintaining appropriate therapeutic boundaries. Community-based program case managers must have a valid FL driver&rsquo;s license.
