About the Role:</b></p><p>
The Home Health Field LVN/LPN plays a critical role in delivering high-quality nursing care to patients within their homes, ensuring comfort, safety, and effective management of health conditions. This position requires the ability to assess patient health status, administer medications, and provide wound care while collaborating closely with physicians, therapists, and family members. The LVN/LPN will be responsible for developing and implementing individualized care plans that promote patient independence and improve overall well-being. This role demands strong clinical judgment and compassionate communication to educate patients and caregivers on treatment protocols and health maintenance. Ultimately, the Home Health Field LVN/LPN contributes to enhancing patient outcomes by providing personalized, attentive care in a home setting, supporting recovery and chronic disease management outside of traditional clinical environments.</p><div>
As a <b>Home Health LVN</b>, you will:</div><ul><li><div>
Under the direction of the RN, assist in identifying the patient&rsquo;s physical, psycho social, and environmental needs as evidenced by documentation, clinical record, team report, and evaluations.</div></li><li><div>
Participate in planning and implementing care in conjunction with the RN, in accordance with the POC.</div></li><li><div>
Applies concepts of infection control and standard precautions in coordinating/performing patient care activities to prevent contamination and transmission of disease.</div></li><li><div>
Assure the continuity of care through delivery of quality patient care.</div></li><li><div>
Provide effective communication to patient/family, team members, and other health care professionals as evidenced by clinical notes, case conferences, communication notes, and evaluations.</div></li><li><div>
Monitor assigned cases to ensure compliance with requirements of third party payor.</div></li><li><div>
Promote Agency philosophy and administrative policies to ensure quality of care.</div></li></ul><div></div><div><b><br>

The Hospice Field Registered Nurse (RN) plays a critical role in delivering compassionate, patient-centered care to individuals facing life-limiting illnesses in their homes or other care settings. This position focuses on managing and coordinating comprehensive nursing care plans that address physical, emotional, and psychosocial needs, ensuring comfort and dignity for patients and support for their families. The Hospice Field RN collaborates closely with interdisciplinary teams, including physicians, social workers, and chaplains, to provide holistic care tailored to each patient&rsquo;s unique situation. This role requires strong clinical expertise, excellent communication skills, and the ability to make independent decisions in dynamic environments. Ultimately, the Hospice Field RN contributes significantly to improving quality of life and facilitating a peaceful end-of-life experience for patients and their loved ones.</p><div>
As a <b>Hospice RN</b>, you will:</div><ul><li><div>
Initial and ongoing comprehensive assessments of the impact of the terminal disease on the patients physical, functional, psychosocial, and environmental needs as evidenced by documentation, clinical record, case conference, team report, evaluations, and ADLs, (i.e., risk for grief, cultural and spiritual, verbal and non-verbal).</div></li><li><div>
Apply specific criteria for admission and re-certification to hospice care to establish appropriate levels of care and the patient&rsquo;s eligibility.</div></li><li><div>
Implement/develop/document the plan of care to ensure quality and continuity of care and recommend revisions to the plan as necessary.</div></li><li><div>
Consult with and educate the patient and family regarding disease process, self-care techniques, end-of-life care, nutrition and dietary needs. Prioritize any needs with the members of the IDG. Provide training to other staff as needed.</div></li><li><div>
Determine scope and frequency of services needed based on acuity and patient/family needs. Assess the ability of the caregiver to meet the patient&rsquo;s immediate needs upon admission and throughout care.</div></li><li><div>
Provide clinical directions to the Hospice Aide and LPN/LVN to ensure quality and continuity of service provided.</div></li><li><div>
Prepare clinical and progress notes that demonstrate progress toward established goals. Ensure continuity of quality patient care delivered with appropriate documentation.</div></li><li><div>
Inform physician and other personnel of changes in the patient&rsquo;s needs and outcomes of intervention, while evaluating patient and family response to care.</div></li><li><div>
Perform on-call responsibilities and provide on-call service to patients and families as assigned.</div></li></ul><div></div><div><b><br>
