A bit about this role:
The Care Coordinator has an important role supporting TOC Clinical Operations as we ensure our patients have appropriate post discharge planning and follow up in place. The Care Coordinator will support the team with patient outreach and coordination while the patient is inpatient and post discharge. 
Your Responsibilities and Impact will include:<ul><li>
Reaching out to patients while inpatient to introduce TOC program and communicate post discharge follow up from TOC clinicians</li><li>
Scheduling post discharge follow ups (PCP/Specialty)</li><li>
Reaching out to hospitals and provider offices to obtain clinical documentation</li><li>
Submitting Home Care and DME orders as well as vendor follow up</li><li>
Monitoring return fax queue and researching provider profile for updated contact and fax information</li><li>
Provide additional operational support for assigned team and manager as designated</li></ul><ul><li>
Ensure patients are prepared for telehealth visits by assisting with technology prep</li><li>
Collaborate internally in clinical rounds to discuss patients at risk for readmission</li><li>
Coordinate lab testing and imaging, and tracking results to ensure that they are reviewed by the ordering clinician</li><li>This role does not include clinical decision-making or any activity requiring licensure, and that all clinical tasks are delegated by, and performed under the supervision of, licensed professionals. Clinical Coordinators should direct clinical concerns raised by patients to the appropriately licensed team.]</li></ul>

Transitions of Care Coordinator

<div>Job Description
This role is a fully remote position. MUST BE LOCATED IN A STATE WE OFFER DEVOTED.
A bit about this role:
As a Transitions of Care (TOC ) Behavioral Health Social Worker, you will focus on Transitions of Care (TOC) support for Devoted members. You will provide short term interventions to members in the process of being connected to community based behavioral health services, either because of an emerging need or following a hospitalization. The goal is to prevent avoidable emergency room/inpatient care and/or prevent readmission to the hospital.
The TOC team is dedicated to providing culturally competent care with a focus on equity and quality. We pay close attention to our team culture, treating each other like family so we have the requisite resiliency to care for our members like family.
Required skills and experience:<ul><li>
Master&rsquo;s degree in Social Work with an active clinical license in any US state - LCSW, LMSW, or comparable state clinical license. Willingness to gain additional state licenses.</li><li>
Minimum of 5 years experience in the behavioral health or care management fields</li><li>
Competent performing comprehensive psychosocial assessments over video and phone</li><li>
Able to independently manage a fully remote work schedule, M-F, 8:30am-5:00pm PST or MST</li></ul>
Desired skills and experience:<ul><li>
Bilingual in English and Spanish, Creole or Vietnamese preferred, but not required</li><li>
Experience in Geriatric BH or the health insurance industry (Medicare Advantage is a big plus)</li></ul>

Performing comprehensive psychosocial assessments via phone/video with members and caregivers</li><li>
Conduct clinical social work interventions to maximize the effectiveness of the program including: brief therapeutic interventions, assistance in navigating family/support issues, assessing and intervening on social determinants of health, completing safety assessments</li><li>
Interdisciplinary collaboration with teammates (including clinicians, nurses, and care coordinators) to ensure cohesive, patient-centered care for high-needs patients</li><li>
Ability to clinically identify and escalate complex members to the BH Medical Director for consultation</li><li>
Actively participate in team huddles, panel reviews, interdisciplinary team rounds, and program/role growth and development</li><li>
Document all clinical interactions in the electronic health record</li></ul>
Salary Range: $55,000 - $79,000 / year

Members with chronic conditions, like hypertension, often lack access to high-quality care because of a scarcity of primary or specialist care in their community or personal barriers to traveling to seek care. Elevated blood pressure is the leading contributor of cardiovascular morbidity and mortality&ndash; in fact, complications from raised blood pressure account for 53% of all heart disease and stroke deaths globally. Despite a mountain of proven effective interventions, the prevalence of hypertension and adverse events related to cardiovascular disease are rising at an alarming rate.
We are on a mission to eradicate preventable death due to hypertension by replicating best-in-class hypertension care through our virtual care platform. Our multidisciplinary team combines remote monitoring, medication management, and high-touch disease education to deliver seamless specialty care in the comfort of our members&rsquo; homes.
As a Cardiovascular Clinical Guide, you will make a meaningful impact every day by supporting members throughout their care journey. Drawing on your coaching skills, you&rsquo;ll review each member&rsquo;s medical history, medications, and lifestyle habits, and help them build the confidence and tools they need to monitor and improve their cardiovascular health. You&rsquo;ll play a key role on our specialty care team, empowering members to set achievable goals, overcome barriers, and make lasting changes that lead to healthier, more fulfilling lives.
Responsibilities will include:<ul><li>
Screen members for eligibility in the Hypertension Clinic</li><li>
Provision BP monitors, assess proper measurement technique, and reinforce accurate self-monitoring practices</li><li>
Conduct comprehensive hypertension assessments, including review of medical history, medications, adherence patterns, and lifestyle habits</li><li>
Deliver intensive lifestyle and nutrition counseling focused on sodium and alcohol reduction, weight management through heart-healthy eating and portion control, increased physical activity, and smoking cessation support</li><li>
Identify barriers to healthy behaviors and coach members toward realistic, sustainable health goals</li><li>
Utilize motivational interviewing skills with the goal to influence positive sustained behavioral changes</li><li>
Maintain accurate and timely documentation of all member interactions</li><li>
Schedule follow-up appointments with yourself, program providers, or the member&rsquo;s primary care provider as needed</li><li>
Connect members with the right resources within Devoted to troubleshoot clinical or care-coordination issues</li><li>
Coordinate multidisciplinary care with Devoted teams, including Specialty Care NPs, Clinical Pharmacists, Social Workers, Behavioral Health, Community Guides (financial assistance), Clinical Associates, and Outbound Guides</li><li>
Serve as the member&rsquo;s primary point of contact throughout program enrollment, guiding them through the care journey</li><li>
Synchronize and communicate care plans with PCP or other community care providers</li><li>
