General Position Description: Responsible for comprehensive coordination of behavioral health services for adult and child members eligible for enrollment with public behavioral health services.
Essential Duties and Responsibilities:<ul><li>
Assesses the member&rsquo;s behavioral health needs and eligibility for services. Assists staff in developing and monitoring member service plans.</li><li>
Provides comprehensive intakes, assessments, annual assessments, and offers therapeutic and support services. Screens, evaluates, and schedules referred members who may need behavioral health services.</li><li>
Coordinates communication between the member and other providers or community agencies to ensure coordination of care. This includes assisting with scheduling appointments, attending telemed appointments, arranging transportation, conducting CFTs and ARTS, and coordinating with the hospital for d/c appointments.</li><li>
Provides rehabilitation services that include living skills training, counseling, and supported employment services to enrolled members.</li><li>

Performs crisis intervention and solution-focused interventions.</li><li>
Maintains EBF productivity, timelines, and quality standards set forth by EBF.</li><li>

Requirements:

Minimum Requirements: High School Diploma/GED with 1 year of behavioral health or related work/volunteer experience or an Associate&rsquo;s/Bachelor&rsquo;s degree in Behavioral Health required. The education and experience level will determine the position level of Behavioral Health Technician (BHT) or Behavioral Health Professional (BHP). Bilingual (Spanish) preferred.
Regulatory<ul><li>
Must be at least 21 years of age.</li><li>
Current, unrestricted AZ driver&rsquo;s license with no more than two (2) minor moving violations or one (1) accident within the past three (3 years). Three (3) years of driving experience required.</li><li>
Transportation for daily visits, to attend trainings, and perform other work-related activities. Organizational vehicles may not be available at all times.</li><li>
Must be able to pass a criminal background check prior to employment and regular checks for the duration of the employment.</li><li>
Ability to obtain and maintain an AZ DPS Level 1 fingerprint clearance card (employer provides).</li><li>

Able to establish and maintain a team atmosphere of communication and collaboration for all that reach out to the Department. Must be self-directed and be sensitive to cultural and linguistic diversity. Must be able to utilize DSM-5-TR to diagnose older children, adolescents and adults and the crosswalk to the DC-0-5 to provide diagnoses or diagnostic impressions for young children. Must remain calm and non-defensive with a supportive attitude during stressful, potential crisis, and crisis situations. Must be able to work in an extremely stressful environment where there are considerable mental and emotional demands. Must be able to manage high volumes and unpredictability of work. Must be able to remain flexible and reprioritize according to the changing demands of the day. Maintains a professional in appearance, communications, and actions. Consults and collaborates with other professionals involved in providing services to women and families.</li><li>
Actively engages in case conferences and continuous quality improvement activities. Elicits and considers differing viewpoints when analyzing issues.</li><li>
Maintains a caseload of 25-30 clients, when working as a full-time nurse home visitor.</li><li>
Participates in the process of ongoing professional development and meetings. Maintains comprehensive communication among all contacts within the agency, members, and external agencies. Models professional standards and ethics in accordance with EBF Code of Conduct, Strategic Plan, Mission, and Vision.</li><li>
Performs other duties as assigned.</li></ul>
Requirements:

Minimum Requirements: Associate Degree in Nursing (ADN) or Bachelor&rsquo;s degree in Nursing(BSN) and current RN license in good standing required. Experience in maternal/child health, public health, home visiting or mental/behavioral nursing preferred. Bilingual (Spanish) preferred.
Regulatory<ul><li>
Must be at least 21 years of age.</li><li>
Current, unrestricted AZ driver&rsquo;s license with no more than two (2) minor moving violations or one (1) accident within the past three (3 years). Three (3) years of driving experience required.</li><li>
Transportation to family homes, sites, attend trainings, and perform work related activities. Organizational vehicles may not be available at all times.</li><li>
Must be able to pass a criminal background check.</li><li>
Ability to obtain and maintain an AZ DPS Level 1 fingerprint clearance card (employer provides).</li><li>
CPR and First Aid certification (Employer provides).</li></ul>
Skills/Job Knowledge/Abilities: Able to establish and maintain a team atmosphere of communication and collaboration for all that reach out to the Department. Able to communicate clearly, effectively, with confidentiality, openness and professionalism with families, colleagues, and stakeholders Able to listen attentively and accurately to the client&rsquo;s current experience. Must be self-directed and be sensitive to cultural and linguistic diversity. Excellent customer service in stressful situations. Maintains a professional appearance, communications, and actions. Basic knowledge of the respective evidenced-based home visitation model&rsquo;s (MECSH or NFP) policies, practices, and procedures. Knowledge of prenatal and post-partum maternal health; the dynamics of intimate partner violence and safety planning; infant/toddler typical and atypical development; family systems and principles of family-centered care; and community health and social service resources. Possess a high level of computer proficiency.
Working Conditions/Physical Requirements (with or without accommodation): Client in-home and office environment with multiple interruptions in person and through computer and phone. Will work outside and inside environments, as needed. Daily travel to client homes and other locations. Interacts with employees and members of the public on a daily basis. May lift normal office equipment and materials up to 25lbs. Visual acuity to read information from computer screens, forms and to assess the wellbeing of clients and children. Able to speak clearly in conversations and general communications. Hearing ability for communication in persona, phone, and/or other electronic methods. Manual dexterity for typing and writing. Able to stoop, squat, reach, pull, push, stretch, ascend and descend stairs, stand and sit for long periods of time. May be required to work additional hours or days depending on circumstances.
