<div><div><div><p>Eliot's Developmental Disabilities division is seeking a <b>Community Supports Case Manager</b>. This division offers support and opportunities for individuals to enrich their lives, obtain housing and employment, develop personal skills and competencies, and enjoy a full range of community activities while developing a network of caring relationships.</p>
<p>
The ideal candidate will have experience working with adults with developmental disabilities and will have the ability to advocate for community supports and services on behalf of the individuals.</p></div></div><div><br>
<div>
<p><b>Responsibilities:</b></p>
<ul>
<li>Assist in completing assessments and Individual Service Plans(ISP)/Plans of Care(POC) for assigned cases in a timely manner and as indicated by DDS and MRC policies and standards.</li>
<li>Ensure individuals are involved in identifying and developing their own goals.</li>
<li>Document daily progress notes for all individuals in accordance with Eliot Policy and DDS/MRC documentation and billing requirements.</li>
<li>Develop and maintain a positive and professional working relationship with individuals, families and providers. This includes maintaining a set schedule with individuals, timely communication and follow up on issues and requests and being on time for appointments, meetings and activities.</li>
<li>Coordinate service delivery for cases assigned i.e., case conferences, Section 8 Housing, SSI and advocate for the consumer with outside agencies and providers.</li>
<li>Provide service linkages to other providers as necessary. Identify and schedule appointments, for/with individuals, as needed or required.</li></ul></div><br>

<div><div><p><b>Qualifications:</b></p>
<ul>
<li>Bachelor's degree or equivalent experience</li>
<li>Experience working with intellectually disabled or brain injured adults.</li>
<li>Ability to manage a client caseload in an outreach setting.</li>
<li>Valid driver's license and a reliable vehicle.</li>
<li>Ability to work a community based position that services the Lawrence, Lowell, Burlington and Arlington areas. Main office located in Lexington MA.</li></ul></div><br>

<div><div><div><p>Eliot is seeking a compassionate and dedicated <b>Behavioral Specialist I</b> to join our dynamic Adult Community Clinical Service (ACCS) team. ACCS provides clinically focused evidenced based interventions to adults with severe and persistent mental illness. Services are person centered and are delivered by a multidisciplinary team of professionals in either residential or community based settings. In this role, you will provide support and guidance to adults facing severe and persistent mental health challenges.</p></div></div><div><br>

<p><b>Responsibilities:</b></p>
<ul>
<li>Provide interventions to clients that focus on problem solving, modeling behavior, optimism and encouragement</li>
<li>Assist the team in providing on-going assessments of client symptoms and their responses to staff interventions</li>
<li>Support clients in their desire for recovery and rehabilitation in the development of their goals</li>
<li>Ensure that services provided are culturally competent, strengths based, and focused on rehabilitation and are inclusive of client preferences.</li>
<li>Provide assistance to clients related to transportation to work sites, daily activities, and appointments</li></ul></div><br>

<li>Bachelor's degree in Psychology, Social Work, or related field</li>
<li>Experience working with adults with mental illness</li>

<div><div><div><p>Eliot is seeking a strong, experienced <b>Forensic Behavioral Health Specialist</b> to join our Program for Assertive Community Treatment (PACT) team. PACT is an evidence-based model that provides comprehensive treatment, rehabilitation, and support to adults with severe and persistent mental illness. This PACT team is designed to work with a specialized population with a history of criminal engagement.</p>
<p>
The ideal candidate will have an understanding of the unique needs and history/background of the population, clinical interventions and have the ability to develop and maintain clear and therapeutic boundaries. A proven experience in community-based work with adults with severe and persistent mental illness is a must.</p></div></div><div><br>
<div>
<p><b>Responsibilities:</b></p>
<ul>
<li>Provide outreach support services to forensic PACT clients and medication supports as needed to clients living independently in the community.</li>
<li>Provide side by side interventions to consumers that focus on problem solving, skills training, modeling behavior, optimism and encouragement. Assist the team and consumers in providing on-going assessment of symptoms and responses to staff interventions.</li>
<li>Promote opportunities to participate in pro-social activities and interpersonal skill development.</li>
<li>Provide assistance to consumers related to transportation to court, work sites, court mandated treatment, probation/parole meetings, daily activities, appointments or as needed/directed by supervisor.</li></ul></div><br>
</div><br>
<div><div><p><b>Qualifications:</b></p>
<ul>
<li>Must have a bachelor's degree in psychology, criminal justice, social work or related field with at least two years of experience with population.</li>
<li>Must have the skills to establish supportive relationships with adults with mental illness who have limited social skills and experiences with histories of unsuccessful community tenure.
