Registered Nurse Ambulatory - Acute Care Surgery Outpatient Office - Full Time

The Fitch unit is a 45 bed, Acute Care Medical Unit, with approximately 120 employees. The average patient census is 40 patients and specialties focusing on sepsis, respiratory, diabetes, GI, oncology, end of life, and some surgical. The Fitch unit provides an outstanding supportive environment and advanced training for its staff. The unit also offers a relaxation room for its employees to decompress and relief stress. The nursing staff is supported by a charge RN and team lead, whose focus is to make this the best place to give and receive care. Many years ago, our staff came up with our unit&rsquo;s own mission and vision. The mission is &ldquo;Everyone Helps Everyone&rdquo; and the vision is &ldquo;Teamwork Makes the Dream Work&rdquo;. About the Job:</b></p><p>
Functions as an integral member of the care delivery team by providing direct and indirect patient care under the direction of a RN and within the scope of practice of a Licensed Nursing Assistant (LNA). Assists patients with activities of daily living, provides for personal care, emotional support and performs more complex clinical skills under the direction of an RN.</p><p><b>

Models Elliot Essentials behavior with patients, peers and all other members of the health care team.</li><li>
Performs or assists patients in the completion of activities of daily living.</li><li>
Utilizes basic knowledge of growth and development to provide age-specific patient care.</li><li>
Provides basic support to patients and families in response to identified psychosocial needs and cultural and religious preferences.</li><li>
Performs clinical tasks within the scope of LNA practice, in accordance with policies and procedures of the Elliot Health System.</li><li>
Reports all observations, symptoms, deviations from normal, and changes in patient&rsquo;s condition to the Registered Nurse</li><li>
Assists others in performing selected patient care activities or tasks as appropriate.</li><li>
Follows all infection prevention/universal precautions policies as appropriate.</li><li>
Incorporates safety principles, policies and procedures into daily practice.</li><li>
Ensures confidentiality of all patients, employee, legal, budget, and company matters. Maintains patient privacy and dignity.</li></ul><p><b>
Phlebotomy</b></p><ul><li>
Performs routine and emergency phlebotomy.</li></ul><p><b>
Telemetry</b></p><ul><li>
Completes rhythm analysis and interpretation and posts rhythm strips at the specified times.</li><li>
Communicates information regarding rhythm changes and vital sign abnormalities to the primary nurse and charge nurse.</li><li>
Demonstrates knowledge of location of patients that are off the unit for testing.</li><li>
Maintains telemetry alarm notification log.</li><li>
Ensures that communication devices are working properly, assigned and distributed to nursing staff.</li></ul><p><b>
Unit Secretary</b></p><ul><li>
Contributes to ensuring effective and efficient communication with visitors, callers, physicians, staff from other departments and others as appropriate.</li><li>
Meets and greets visitors to the department; determines the purpose of visit and responds or refers to appropriate staff member.</li><li>
Updates and maintains patient assignment board, Census Change Sheets, and all other patient lists.</li><li>
Supports effective and efficient patient care processes through timely, accurate transcription and entry of orders/data.</li><li>
Maintains Unit Secretary task list to ensure patient care which are verified by the registered nurse, to appropriate patient care documents.</li><li>
Assembles patient charts for all admissions to the unit to include all appropriate documents</li><li>
Prepares patient charts for patient transfers by copying and organizing required documents according to specific payer criteria.</li><li>
Disassembles and reorganizes charts upon discharge before forwarding to medical records.</li><li>
Coordinates completion of patient death certificates by contacting appropriate parties.</li><li>
Supports the smooth functioning of the unit by performing a variety of clerical and coordinating activities as designated by the unit.</li><li>
Updates and maintains the daily communication device list for the unit; ensures that all RNs receive communication device at the beginning of the shift and return them at the end of the shift.</li></ul><p><b>
Radiation Therapy</b></p><ul><li>
Schedules, tracks and assures timeliness of diagnostic testing. Obtaining insurance authorization for diagnostic testing.</li><li>
Schedules all follow up clinic appointments.</li><li>
Reviews patient&rsquo;s charts to ensure all necessary information is obtained prior to patient visit.</li><li>
Obtains an updated medication list for all follow-up patients. Coordinates needed services/referrals for follow up patients.</li><li>
Tracks patients missed appointments, and notification to patient to reschedule and sending a certified notice for repeated missed appointments, per protocol.</li><li>
Assists physicians with patient physical exams/procedures/preparation of needed equipment.</li><li>
Ensures all exam rooms are adequately stocked and cleaned. Checking for and replacement of any expired items. Responsible for lined inventory and distribution within the department.</li><li>
Coordinates weekly treatment visits for physicians and RNs.</li><li>
Orders all departmental needed clinical supplies in the Lawson system. Tracks supply of bulk stores and MDS items and makes recommendations to adjust par levels.</li></ul><p><b>
Telesitter</b></p><ul><li>
Maintains constant visual observation of the patients receiving remote visual monitoring.</li><li>
Participates in the hand-off, during shift change or when admitting new patients to the system, of pertinent information/behavior about patients being monitored.</li><li>
Ensures another telesitter in place before leaving monitoring station.</li><li>
Verbally redirects patients when necessary.</li><li>
Provides patients with explanations as necessary, but does not counsel or provide opinions.</li></ul><p></p><p><b>
Who You Are:
</b></p><ul><li>Current Nursing Assistant License in the State of New Hampshire required. BLS authorization preferred or obtained within 90 days of date of hire.</li><li>
High school diploma or equivalent preferred.</li><li>
Minimum of 6 months experience as an LNA</li><li>

The Elliot NICU is the only Level III Newborn Intensive Care Unit in southern New Hampshire, taking care of babies 24 weeks in gestation to term who are critically ill using high-tech resources and a team of dedicated professionals specializing in premature infant care. Our state-of-the-art facility houses 26-beds (isolettes), where we treat more than 400 critically ill and premature infants each year.</p><p></p><p><b>
About the Job:</b></p><p>

Our UCCs are fast paced settings with a well-rounded compliment of LNA&rsquo;s, Nurse&rsquo;s, Physicians and Advanced Care Providers. All three Urgent Care Centers are well established in their communities as a site focused on meeting the non-emergency needs of the community when Primary Care is not available. We pride ourselves on being able to care for very diverse populations, seeing patients from many cultures and across the entire age spectrum. Our nursing model is a team oriented and collaborative. All three locations have a limited menu of unit-based point of care Lab tests run on site for diagnosing common non-life-threatening illness. We also have on site radiology and CT scanning available if needed and Ultrasound is available 8a-4p Mon-Fri with after hours and weekend Ultrasound coverage available at River&rsquo;s Edge.</p><p><b>
About the Job:</b></p><p>
As an LNA at Urgent Care, your primary responsibilities will include providing patient care in accordance with the nurse&rsquo;s instructions, assisting in developing and implementing patient care plans, maintaining medical records documentation, transporting patients to other health care areas as needed and providing emotional support to patients. You&rsquo;ll also be responsible for administering treatments and medications under the direction of a nurse, assessing vital signs such as temperature, pulse, respiration rate and blood pressure; delivering specimens to the laboratory; assisting with physical therapy treatment modalities; and updating patient charts to ensure accuracy in all documentation.</p><p><b>
