The Coder 2 analyzes clinical documentation; assign appropriate diagnosis, procedure, and levels of service codes; abstract the codes and other clinical data. Performs a variety of technical functions within the Outpatient coding area, codes outpatient visits, sent-in-labs, consolidated funding accounts, utilizing ICD-10-CM, CPT-4, and HCPCs Coding Classification systems. Utilizes an electronic coding software to code to the highest level of specificity, ensuring optimal and appropriate reimbursement for the services provided. Responsibility includes resolving medical necessity edits and extracting and entering data into the medical record. This information is then used to determine reimbursement levels, assess quality of care, study patterns of illness and injuries, compare healthcare data between facilities and between physicians, and meet regulatory and payer reporting requirements. Coder 2's also resolves clinical documentation and charge capture discrepancies and provides feedback to providers on the quality of their documentation and charging.

Actively participates in creating and implementing improvements.</li><li>
Assigns ICD-10, CPT-4, and HCPCs codes to all diagnoses, treatments, and procedures, according to official coding guidelines.</li><li>
Knowledge of relationship of disease management, medications and ancillary test results on diagnoses assigned.</li><li>
Extracts required information from electronic medical record and enters encoder and abstracting system.</li><li>
Follows-up on unabstracted accounts to assure timely billing and reimbursement.</li><li>
Resolves any questions concerning diagnosis, procedures, clinical content of the chart or code selection through research and communication. May query physicians on documentation according to established procedures and guidelines.</li><li>
Meets departmental productivity and quality standards</li><li>
Complete projects as assigned.</li><li>

Required Qualifications</b></div><ul><li>
Certificate program in Coding or A.A./A.S. in HIM or Certificate with 1-3 years of healthcare experience (MA, HUC, Revenue Cycle)</li><li>
1 year of coding experience</li><li>
Basic knowledge of Windows-based computer software. Epic and Microsoft Teams.</li><li>

Registered Health Info Admin (RHIA) or</li><li>

M Health Fairview is posting a position for a 0.6 FTE Palliative care physician who will be working in the University and community hospitals. The Palliative Care Physician will be a member of the palliative care consultation team and will work in collaboration with other physicians, APPs, palliative care social workers and chaplains to provide timely access and care to patients with palliative care needs. They will provide all necessary acute care services to patients at M Health Fairview, provide timely consultations to requesting clinicians in different settings, ensure proper care, diagnosis and treatment as necessary. The scope of practice of each Palliative Care Physician is defined through the Fairview's Credentialing and Privileging Committees. The Palliative Care Physician is responsible for the care of patients by performing thorough examinations, assessments, and diagnostic evaluations and then development of treatment plan. For this 0.6 FTE position, the palliative care physician's hospital workdays will be proportional to their 0.6 FTE and will be on a rotational basis. Weekend call will also be proportional to the 0.6 FTE.</div><div></div><div><br>
Responsibilities include:</div><ul><li>

Ensure patients receive appropriate care according to their condition, symptoms, and diagnoses. Develop a care plan in conjunction with other physicians and interdisciplinary staff to ensure quality of care. Complete effective and timely communication with coordinating physicians and interdisciplinary staff during the patient's various stages of care.</li><li>
Communicate with patients and families to explain diagnoses, diagnostic or therapeutic treatments, procedures, and prognosis, as well as discuss potential or expected outcomes. Communicate to primary and/or referring physicians. Proactively communicate with other clinicians to effectively coordinate care and timely transfer of information necessary to improve the quality of patient care.</li><li>
Document patient consultation, treatment plan, orders, and clinical progress in the medical record. Dictate, complete records, and submit charges by the deadline as defined by system policy. Returns pages, phone calls, and EMR (Electronic Medical Record) tasks in a timely manner.</li><li>
Work closely with the Medical Director and Clinic Manager to ensure M Health Fairview patient care meets and/or exceeds established requirements. Cooperate and participate in the development of quality improvement initiatives. Attend monthly team meetings, huddles, and other meetings as assigned</li></ul><div>
Required Qualifications</div><ul><li>
Doctorate (M.D.) or</li><li>
Doctor of Osteopathic Medicine</li><li>
Graduate of an accredited ACGME training in Hospice and Palliative Medicine</li><li>
Board Certified/Board Eligible in Hospice and Palliative medicine</li><li>
Licensed or ability to obtain Minnesota Board of Medical Practice Licensure</li><li>
Ability to obtain and/or maintain DEA certification</li><li>
