We are looking for a motivated and compassionate Band 3 Discharge and Flow Coordinator to join our friendly Discharge and Flow team, based at Paulton Memorial Hospital. This is a rewarding opportunity to make a real difference to patients and their families by supporting safe, timely discharges into community services across our BSW Community Hospitals.
In this role, you will work closely with nurses, therapists, social workers and other members of the multidisciplinary team to coordinate patient flow and ensure everyone has the right information at the right time. You will play a key part in keeping discharge processes running smoothly, maintaining accurate records, completing referrals and supporting clear communication across services.
You&rsquo;ll also build positive relationships with patients and their families on the wards, helping them understand the discharge process and keeping them informed throughout their stay.

Paulton Memorial Hospital is located in the heart of the village of Paulton, set in the beautiful Somerset countryside close to the Mendip Hills. Main Responsibilities</h4><ul><li>
Support safe and timely patient discharges by completing all required administration early and accurately.</li><li>
Visit patients on the wards to keep them and their families updated on discharge plans.</li><li>
Help people understand the discharge process, expected timeframes and the role of the Flow team.</li><li>
Attend MDT meetings and record actions, updates and discharge readiness on clinical systems.</li><li>
Attend professional and family meetings and capture clear, accurate minutes.</li><li>
Monitor predicted discharge dates for all patients and highlight changes or concerns to the team.</li><li>
Make sure patients are registered on the correct clinical systems and that electronic records are kept up to date.</li><li>
Use multiple digital systems confidently to support smooth, timely discharges and avoid delays.</li><li>
Identify patients who may need complex discharge planning and raise this with the Senior Coordinator.</li><li>
Ensure all referrals are completed before discharge and escalate where actions are outstanding.</li><li>
Maintain excellent communication with the MDT, system partners and commissioners.</li><li>
Provide clear information leaflets to patients, families and carers when needed.</li><li>
Answer calls and relay messages in a professional, respectful and accurate manner.</li><li>
Collect data and support required reporting for the service.</li><li>
Promote good communication within the MDT and make sure predicted discharges are shared daily.</li><li>
Act as a point of information on available services and referral pathways.</li><li>

NVQ Level 3 in Care, Business Administration or equivalent experience</li><li>

We are seeking a motivated and compassionate Engagement Support Officer to join the 0-19 Inclusion Health Team, dedicated to improving the health and wellbeing of some of the most vulnerable children, young people, and families across Surrey.
This role plays a vital part in delivering a specialist, enhanced Health Visiting service to communities experiencing homelessness and social exclusion, including asylum seekers, refugees, families in emergency accommodation, and those from marginalised or nomadic backgrounds such as Gypsy, Roma and Traveller communities.
As a key member of the team, you will support the delivery of targeted health interventions through assertive outreach and meaningful community engagement.
Working in partnership with statutory services, voluntary organisations, and local community groups, you will help reduce barriers to accessing healthcare, address health inequalities, and strengthen connections between underserved populations and essential services.
The successful candidate will collaborate closely with clinical colleagues, vaccine equity coordinators, immunisation providers, and wider partners to promote public health initiatives, including increasing uptake of routine immunisations among vaccine-hesitant and underserved communities. Through innovative and culturally sensitive approaches, you will contribute to improving health outcomes and empowering individuals and families to engage with services confidently.
You will play an active role in system-wide collaboration across the Integrated Care System (ICS), fostering partnerships with services such as mental health, sexual health, and other outreach teams to deliver a joined-up approach to care.
This position also involves developing creative engagement strategies, producing accessible resources, and using digital, media, and visual tools to connect effectively with diverse communities.
