JOB TITLE: Psycho Social Rehabilitative Specialist.DEPARTMENT: COREREPORT TO: CORE DirectorLOCATION: ONSITEWe believe that in-person operations are the best way to serve our clients, both directly and through essential operational support. This position requires in-person attendance at 5 different Brooklyn clinics with travel to the Bronx, Manhattan, and Queens to support CORE Services.Core Services includes:<ul><li>Crisis mental health services including 24-hour mobile crisis teams, emergency crisis intervention, and crisis stabilization; Screening, assessment, and diagnosis including risk assessment; Patient-Centered treatment planning or similar processes, including risk assessment and crisis planning; Outpatient mental health and substance use services; Outpatient clinic primary care screening and monitoring of key health indicators and health risk; Psycho Social rehabilitation services; Peer support, counselor services, and family support services.</li></ul>SUMMARY: CORE Services. The Psycho Social Rehabilitative Specialist (PRS) provides recovery-oriented, community-based mental health services to individuals with serious mental illness/substance abuse. In this role you will support clients in developing skills, accessing resources, and achieving personal goals related to independent living, employment, education, social functioning, and overall wellness.Your contributions will directly support our mission of enhancing community well-being.JOB DESCRIPTION: <ul><li>Provide psychosocial rehabilitation services focused on skill-building, recovery, and community integration to children, youth, adults and their families.</li><li>Assist children, youth, adults and their families in developing and achieving individualized person centered recovery goals.</li><li>Assist, support and reinforce daily living skills, including self-care, personal hygiene, budgeting, medication management, time management and more to children, youth, adults and their families.</li><li>Support the development of personal autonomy, self-advocacy, wellness, recovery and independent decision-making.</li><li>Support children, youth, adults and their families in identifying their rights, needs, and preferences.</li><li>Assist children, youth, adults and their families with dietary education which in compasses but not limited to healthy meal planning, shopping and preparation.</li><li>Assist children, youth, adults and their families in accessing their community such as transportation, route planning, time awareness and identifying transportation resources to increase independence.</li><li>Assist with community integration activities, including building natural supports and social networks.</li><li>Assist with increasing community awareness and community tenureship.</li><li>Assist in facilitating effective communication and interactions with health care professionals.</li><li>Assist children, youth, adults and their families in managing mental health /substance use symptoms, and medication routines in coordination with treatment teams.</li><li>Provide psychoeducation related to mental health, recovery, wellness, and self-management.</li><li>Support benefits navigation and basic financial management skills with practice and coaching activities.</li><li>Assist individuals in connecting to the development of employment goals.</li><li>Assist individuals in connecting with school and achieving academic goals.</li><li>Observe and report changes in mental status, functioning, or risk, and communicate concerns to supervisors.</li><li>Collaborate with licensed clinicians, psychiatrists, nurses, and care coordinators.</li><li>Provide crisis prevention support and assist with de-escalation strategies as needed, following IDCC protocols</li><li>Participate in ongoing training, supervision, and professional development activities.</li><li>Complete all assigned PSR training at onboarding and as required to maintain PSR skills.</li><li>Collaborate with an interdisciplinary team which includes therapists, care coordinators, other certified peer specialists, mobile crisis clinicians, psychiatric nurse practitioners and psychiatrists to assist with an integrated person-centered goal planning.</li><li>Communicates to the supervisor about the need for proper treatment and referral protocols with respect to child abuse, domestic violence, consultation with specialty clinics, and referral both within and outside of IDCC.</li><li>Follow policies, procedures, and protocols to ensure compliance with CORE program requirements and regulations for substance abuse services.</li><li>Complies with billing and reimbursement requirements, maximizing revenue potential while maintaining program integrity.</li><li>Comply with all relevant regulations, accreditation standards, and reporting requirements.</li><li>Participate in audits/reviews to ensure adherence to program requirements and standards.</li><li>Complete agency training to improve cultural competency and trauma informedcare as they align to CORE program and Agency specific mission.</li><li>Perform other work-related duties as assigned (facilitate groups).</li><li>Available to travel/work evening and weekend hours as required by the agency.</li><li>Work collaboratively with the CORE Interdisciplinary Team as well as external agencies and community stakeholders.</li><li>Attend required management and staff meetings.
