We are seeking a motivated and enthusiastic Care Coordinator to join our Working Age Community Mental Health Service. This is an excellent opportunity for someone committed to supporting service users in their recovery within a community setting.
 
Our Community Mental Health Teams operate Monday to Friday, 9am–5pm, and we offer hybrid and flexible working arrangements to support a positive work–life balance.
 
As a key member of our multidisciplinary team, you will provide a range of evidence‑based interventions for working‑age adults experiencing mental health difficulties. We are looking for individuals who are organised, proactive, and able to work both independently and collaboratively. You will bring strong risk assessment skills, a recovery‑focused approach, and a solid grounding in therapeutic interventions. Central to your role will be working with service users to ensure their recovery goals inform their formulation and care planning.
 
This is an exciting time to be part of our service. We are preparing for the Community Transformation Programme, a national initiative aimed at enhancing mental health care for people with severe and enduring mental illness (SMI).
 
You will have the unique opportunity to work alongside experts by experience, third‑sector partners, primary care, and adult social care, contributing to service improvements that address health inequalities and better meet the needs of our communities. With access to appropriate clinical and management supervision the post holder will have or will develop and will be able to demonstrate in practice a range of appropriate skills to include assessment (including risk assessment), formulation, safety planning and clinical interventions.
 
The role provides opportunity to coordinate the care of service users, throughout their engagement with the service and wider services if these are required. To achieve this the post holder, through a range of defined competencies, will acquire all of the skills necessary to carry out role. This will include a requirement to practice care co-ordination skills through shadowing opportunities and supervision. Workload will be continually negotiated through supervision and case load management with the appropriate clinical team managers.

Care Coordinator

The Rough Sleepers Mental Health Team support a number of clinical and medical staff who deliver mental health care and treatment within the community. The service is looking to recruit a secretarial staff member into their service. Working within this role, you will provide comprehensive secretarial support for the clinical teams.
 
You will be required to have a strong and professional work ethic, have excellent communication, organisational, and customer service skills, and be able to multi-task to a high level. There is an expectation that you will be highly IT literate. Previous experience of audio typing and minute taking is essential, as both duties will form a significant part of this role.
 
You will be required to liaise with colleagues in other teams within the care group, and you would need a flexible approach to your work, under the guidance of the Team Coordinator.
 
We need someone with excellent administration/secretarial skills, who is adept at dealing with the challenges thrown up by supporting a busy service.
 
You must be willing to work flexibly within a challenging environment and be able to demonstrate good written and verbal communication, inter-personal, and administration skills.
 
Responsibilities will include, but are not limited to, producing and sending letters to service users and professionals, audio typing using the Trust’s BigHand system, operating the patient CareDirector system, minute taking of team and Trust meetings, managing email inboxes, ordering supplies, and other ad hoc tasks.
 
You will have good IT skills with experience of MS Word, Excel, and PowerPoint. There is also the expectation that you will provide cover for colleagues at short notice on your working day if required, as instructed by the Team Coordinator.
