Summary</b><p><br>
Provides care under the direction and supervision of a physical therapist. Teaches patients exercise for mobility, strength, and coordination; trains on activities such as walking with crutches, canes, or walkers; teaches massage; demonstrates the use of physical agents and electrotherapy such as ultrasound and electrical stimulation. Measures changes in the patient's performance as a result of the physical therapy provided.

<b>Job Duties</b></p><ul><li>
Provides appropriate physical therapy treatment consistent with the plan of care and the patient's condition to restore function, prevent disability, and help patients reach their maximum performance level.</li>
<li>Uses appropriate teaching methods and materials to instruct patients, families, and/or staff in exercise or mobility techniques or the use of equipment.</li>
<li>Recommends modifications to existing physical therapy treatment plans to achieve maximum results.</li>
<li>Applies modalities as indicated to aid patients in recovery and/or pain relief.</li>
<li>Provides cost effective care in accordance with departmental billing guidelines.</li>
<li>Completes and submits all documentation based on policies/standards and regulatory requirements.</li>
<li>Communicates regularly with supervising PT regarding patient concerns and progress.</li>
<li>Select and fits appropriate assistive devices for gait training and instructs patient in proper use.</li></ul><br>

Minimum Qualifications</b><ul><li>
Associate&rsquo;s Degree in physical therapy assisting.
</li><li>Ability to train and mentor others.

Lehigh Valley Health Network (LVHN) is growing to serve our community. More than 2,000 primary care and specialty physicians, as well as more than 800 advanced practice clinicians, are employed by the health network through LVPG.</p><p></p><p>
This position is located in Nazareth, Pennsylvania. This practice currently has 7 physicians and 2 APC's. The office is open Monday-Friday 8a-5p with call being rotated between all the clinicians in the practice. Patient slot durations consist of 40 minutes for new, wellness/preventative for 45 and older, TOC, and medical clearance and 20 minutes for return patients, wellness/preventative for 45 and under, and acute visits. Full time is considered 36 patient facing hours with part time being considered.</p><p></p><p>
The Division of Internal Medicine is growing with more than 20 practices and 100 clinicians.</p><p>

Lehigh Valley Health Network (LVHN) is growing to serve our community. More than 2,000 primary care and specialty physicians, as well as more than 800 advanced practice clinicians, are employed by the health network through LVPG.</p><p></p><p>
This practice is located in Allentown Pennsylvania in Lehigh County. At this location, there are 13 Physicians and 4 APCs. The office is open 8a-5p with call being rotated between all the Physician's and APCs. Patient slot durations consist of 40 minutes for new, wellness/preventative, TOC, and medical clearance and 20 minutes for return patients and acute visits. Full time is considered 36 patient facing hours with part time being considered.</p><p>
The division of internal medicine is growing with more than 20 practices and 100 clinicians.</p><p>

This practice is located in Gilbertsville Pennsylvania in Montgomery County. At this location, there are 2 Physicians and 1 APCs. This practice is looking to have both Family and Internal Medicine. The office is open Monday-Friday 740a-5p with call being rotated between all the Physician's and APC's in the region. This health center also shares the building with Cardiology, OBGYN, Orthopedics, Pediatrics, and Pulmonology. This practice also has a micro hospital in the same parking lot. Patient slot durations consist of 40 minutes for new, wellness/preventative for 45 and older, TOC, and medical clearance and 20 minutes for wellness/preventative for under 45, return patients and acute visits. Full time is considered 36 patient facing hours with part time being considered.</p><p>

<b>Summary</b><br>
Coordinates all patient scheduling activities including appointments, procedures, and exams for multiple hospital, physician practice, outpatient departments, modalities, and facilities. Facilitates communication between staff, referral sources and physician offices regarding concerns/issues related to scheduling.<br>

<b>Job Duties</b></p><ul><li>
Collects clinical information, obtains insurance, and verifies patient demographics to ensure appropriate scheduling of screening and diagnostic tests.</li><li>
Schedules patients for physician appointments and diagnostic test procedures.</li><li>
Educates patients, families and physician offices regarding department protocols, procedures, insurance, referrals and testing criteria.</li><li>
Monitors practice protocols and department procedures and makes recommendations for improvement.</li><li>
Utilizes scheduling software and other patient-specific software applications to facilitate patient access into the network.</li><li>
Serves as a resource for staff, physicians, and referring offices.</li><li>
Strives to reduce errors, minimize rework and defects through conscientious defects and attention to detail.</li></ul><p><b><br>
Minimum Qualifications</b></p><ul><li>
High School Diploma/GED</li><li>
6 months medical office, call center or customer service oriented work experience or</li><li>
in lieu of work experience, the completion of an Associate's Degree program or higher will be accepted.</li><li>
Ability to maintain strict level of confidentiality.</li><li>
Ability to work in a fast paced environment while handling multiple responsibilities.</li><li>
Detail-oriented with the ability to enter information accurately into electronic systems.</li><li>
Demonstrates ability to handle stressful situations.</li><li>
Proficient in Microsoft Office applications.</li></ul><p><b><br>
Preferred Qualifications</b></p><ul><li>
Familiarization with EMR and scheduling systems such as EPIC, PHS etc.</li><li>
Bi-lingual English/Spanish.</li></ul><p><b><br>
Physical Demands</b><br>
Lift and carry 25 lbs. frequent sitting/standing, frequent keyboard use, *patient care providers may be required to perform activities specific to their role including kneeling, bending, squatting and performing CPR.<br>
