Case management and behavioral health services are fully integrated into our care to address various medical and psychosocial factors impacting the patient's overall health. As a Federally Qualified Health Center in Northern California with over 45 years of dedicated service to the community, LifeLong serves over 66,000 patients in the East Bay Area's socio-economically underserved regions. We provide medical, dental, behavioral health, and school-based services at over 15 primary care sites across Oakland, Berkeley, and West Contra Costa County. From opening a residential respite program for homeless persons discharged from the hospital to offering free COVID testing to the community, we are innovative and responsive to our times' social and medical needs. </b></div><div><b>Benefits:</b></div><p>
Compensation:<b> $203,000-$225,000/year</b> for MD/DO and pro-rated to FTE and depending on years since licensed, <b>sign-on bonus,</b> and excellent benefits: medical, dental, vision, Flexible Spending Accounts, dependent and domestic partner coverage, 403(b) retirement savings plan, and loan repayment programs. LifeLong is a qualified site for federal and state loan repayment programs <b>(HPSA score is 20).</b> Acceptance rates may vary year to year based on funding levels and the number of applicants.</p><br>

Responsibilities:
<ul><li>Provides preventive and primary health services in a Community Health Clinic setting.</li><li>
Acts as advocate for clients.</li><li>
Consults with other health team members including case managers.</li><li>
Maintains client records necessary for clinic reporting and billing.</li><li>
Performs history and physical examinations of patients, assess clinical status of patients, and establishes and maintains medical records.</li><li>
Orders and/or performs laboratory and screening tests; request outside consultations and evaluations as needed.</li><li>
Plans a program of care and formulates problem lists based on clinical findings; recommend treatment and medication; provide follow-up care.</li><li>
Provides education, verbal and written, as well as counseling to patients, families, and caregivers.</li><li>
Recognizes and evaluates situations which call for immediate attention of a physician and institute, when necessary, treatment procedures essential for the life of the patient.</li><li>
Supports and or participates in triage functions both by phone and walk-in.</li><li>
Other duties as assigned.</li></ul>
Qualifications:
<ul><li>Commitment to the provision of primary care services for the underserved with demonstrated ability and sensitivity in working with a variety of people from low-income populations with diverse educational, lifestyle, ethnic, and cultural origins.</li><li>
Ability to take the initiative, work independently and make sound judgments within established guidelines; understand and apply oral and written instructions; establish and maintain effective working relations with staff, clinical providers, managers, and external agencies or organizations.</li><li>
Excellent interpersonal, verbal, and written skills and ability to effectively work with people from diverse backgrounds and be culturally sensitive.</li><li>

Strong organizational, administrative, multi-tasking, prioritization, and problem-solving skills.</li><li>

 <b>Job Requirements</b></div><ul><li>
Current California MD, DO or NP/PA license.</li><li>
Training and experience in geriatrics, internal medicine, and/or family practice with board eligibility or certification required.</li><li>
Interest in working with a diverse patient population in a community-based organization and sensitivity to their needs and health problems; commitment to a community health philosophy of prevention and primary care.</li><li>
Experience with electronic health records.</li></ul><div></div><div><br>
