<div>As a Pharmacy Technician in our community pharmacy, you will play a vital role in supporting both the pharmacy team and the patients we serve. Your responsibilities will extend beyond traditional prescription dispensing to include patient-centered, proactive care. ESSENTIAL DUTIES AND RESPONSIBILITIES
Prescription Management:<ul><li>
Accurately process, fill and label prescriptions for dispensing under the supervision of a pharmacist.</li><li>
Initiate, follow-up, and obtain Prior Authorizations.</li><li>
Prepare, send, and receive orders for medications and supplies; maintain inventory, ensuring medications are purchased and stored properly.</li></ul>
Patient Engagement:<ul><li>
Identify and enroll patients in medication synchronization (med sync), ensuring all medications are prepared for pick-up on a single date each month or 90 days.</li><li>
Conduct monthly follow-up calls with patients to confirm medication needs, discuss changes in medication regimens, and identify potential clinical interventions like immunizations.</li></ul>
Clinical Support:<ul><li>
Assist pharmacists in delivering enhanced clinical services, including immunizations, medication therapy management (MTM), and chronic disease management.</li><li>
Document patient care activities and coordinate with the broader healthcare team to ensure continuity of care.</li><li>
Engage in proactive care strategies, identifying potential health issues early and working with pharmacists to implement preventative care measures.</li><li>
Obtain certification to administer immunizations, expanding your role in providing supervised direct patient care.</li></ul>
Workflow Optimization:<ul><li>
Contribute to the pharmacy&rsquo;s shift from reactive to proactive patient care by adopting best practices for workflow innovation, including appointment-based models.</li><li>
Participate in continuous quality improvement initiatives aimed at enhancing patient care and operational efficiency.</li></ul>
Financial and Operational Tasks:<ul><li>
Maintain petty cash and change fund balances.</li><li>
Proficiently handle all transactions at the point of sale.</li><li>
Perform other duties as assigned under the direction of the pharmacist.</li></ul>
EDUCATION AND/OR EXPERIENCE
High School Diploma or equivalent preferred
Graduate of a pharmacy technician program preferred. Experience in a community pharmacy setting is preferred.
CERTIFICATION/LICENSURE
Pharmacy Technician Licensure required
Certified Pharmacy Technician (CPhT) or Certification through NHA required.
OTHER SKILLS AND ABILITIES
Effective communication and customer service skills
Ability to multitask and manage time effectively in a fast-paced environment
Proficiency in using pharmacy management software and technology tools.
Willingness to obtain immunization certification.

Certified Pharmacy Technician

<div>ESSENTIAL DUTIES AND RESPONSIBILITIES<ul><li>
Provides psychosocial assessment, intervention and facilitates referrals to community resources to hospital patients in a variety of care settings.</li><li>
Formulates, develops and implements psychosocial treatment plan utilizing appropriate social work interventions which may include crisis intervention and grief and bereavement services.</li><li>
Assesses referred patients, within prescribed time frames, for transitional planning needs, including any barriers to discharge, family and social supports, capacity for self-care, adaptive abilities and financial resources.</li><li>
Develops individualized, patient-focused plan for meeting continuing care needs.</li><li>
Provides education and resource options available to patients and families. Facilitates and assists with legal issues within scope of practice (POAHC, Guardianship, Patient Rights, and Adoption).</li><li>
Reviews advance directives and helps patients to complete new documents.</li><li>
Communicates effectively with physicians, patients, families and multidisciplinary team to ensure that the patient's right to self-determination is met.</li><li>
Provides education and support to patients, families in regard to issues related to adaptation to the patient's diagnosis, illness, treatment and or life situation.</li><li>
Maintains detailed knowledge of community resources to effectively address patient needs and help to strengthen patient's network of support.</li><li>
Documents plan of care, interventions, and progress toward goals in the medical record in a timely and concise manner.</li><li>
Facilitates crisis intervention services in the hospital setting as determined by scope of practice.</li><li>
Identifies and reports child and or elder abuse to local authorities according to law.</li><li>
Acts as a liaison with county officials for patients involved with protective services, aging, developmental disabilities, domestic violence, mental health services and economic support.</li><li>
Maintains current knowledge and awareness of payer, reimbursement practice and regulations impacting the patient's plan of care.</li><li>
Negotiates care issues as appropriate (indigent issues and availability of resources).</li><li>
Promotes customer satisfaction and intervenes at critical moments of service.</li><li>
Monitors inpatient readmissions for potential failed discharge plans or need for specialized intervention.</li><li>
Provides follow-up with patients as needed.</li><li>
Assists with identifying avoidable day information for organizational tracking and trending.</li></ul>
EDUCATION AND/OR EXPERIENCE
Bachelor's degree from accredited college or university (BSW or equivalent) required. One to two years related experience and or training in acute care setting is desirable. 
CERTIFICATION/LICENSURE
LSW or CSW in state of practice required.<ul><li>We are also open to higher levels of Social Work licensure.</li></ul>
Case Management Certification (ACMA) preferred. 

 
OTHER SKILLS AND ABILITIES 
Phone Skills 
Must be skilled with use of voice mail and phone features. Must be able to use available technology for language interpretation. 
 
Computer Skills 
Must have expert knowledge of Allscripts Care Management program and Epic EMR. Must be able to use Microsoft Outlook; and have basic knowledge of Excel and Word. Must be able to toggle between computer applications. 
 
Must be available to work and or be on-call during weekends and holidays, as assigned. On-call requires accessibility by pager or cell phone during assigned hours and ability to respond promptly to pages by phone. 
 
LEVEL OF SUPERVISION 
Assignments are based on abilities and are normally received in the form of results expected and due dates. Only general procedures are available to guide work. Must be able to work independently with minimal guidance

<div>ESSENTIAL DUTIES AND RESPONSIBILITIES
Proficient with Hospital Information System-HIS and Laboratory Information Systems<ul><li>-LIS to release orders or register patients as needed.</li><li>
Performs patient registration, payment collection, data entry of demographic and insurance information and other procedures as necessary to support efficient sample processing.</li><li>
Transfers patient test requests from scripts, requisitions, and verbal orders into the HIS and laboratory computer systems.
</li><li>Answers and screen telephone calls in a professional and helpful manner.</li><li>
Resolve unusual or unclear test orders by contacting the physician, nurse, reference laboratory and documents actions taken. Refer all unresolved issues to the appropriate supervisor.</li><li>
Generates lists and labels. Runs daily reports.</li><li>
Reviews order monitor and investigate the collection summary report.</li><li>
Assess orders for special collection requirements; patient preparation and appropriate order of draw.</li><li>
Accurately identify patients using two patient identifiers. Proficiently perform phlebotomy on patient population to obtain blood specimens.</li><li>
Adheres to safety protocols and infection control practices at all times.</li><li>
Possess the ability to assess patient response to procedure and interact with the patient at a level that is understandable and comfortable for the patient. Assess patient reaction to procedures and adjust process as indicated. Obtain medical and supervisory assistance as necessary. Accurately identify and label specimens with patient name, date of birth, date and time of collection and collector code.</li><li>
Assure specimen integrity by using aseptic technique and following appropriate procedures.</li><li>
Proficiently collect other samples as indicated including throat swabs, MRSA swabs, urine samples, heel sticks, finger sticks, blood cultures and blood bank specimens.</li>
Performs arterial punctures to obtain blood specimens from adults including the appropriate collection charge.<li>-RMH Phlebotomy only</li><li>
Performs glucose tolerance and hormone stimulation specimen collection in the specified time increments, as well as accurate patient preparation.</li><li>
Performs research program, Lab One, and special collection kit specimen collection.</li><li>
Provides patient instruction in the collection and handling of personally collected specimens.</li><li>
Performs specialized sample collections including breath alcohol testing, random urine drug screens, pre-employment urine drug screens and legal urine drug screens.</li><li>
Receives and accessions collected specimens using LIS, examines for testing acceptability, centrifuges and aliquots specimens as needed and delivers to the appropriate section in the laboratory or batches specimens for courier pick-up.</li><li>
Appropriately notify testing personnel of stat requests.</li><li>
Accept specimens from a variety of internal and external customers; sort samples; resolve specimen identification or requisition issues; utilize LIS to appropriately receive specimens; package and track specimens for transportation to reference laboratories.</li><li>
Possess working knowledge of insurance processes specific to reference laboratories to assure billing is done in accordance with government regulations.</li><li>
Is knowledgeable of general insurance information as related to registration and basic rules related to billing such as proper use of collection dates.</li></ul><ul><li>
Drives laboratory personal vehicle to client sites, patient homes or facilities. Performs accurate specimen collection at client sites, patient homes or facilities. Transports specimens properly to the lab. Assure receipt of samples to the laboratory;Performs limited waived testing including blood glucose monitoring according to established procedures.</li><li>
At the discretion of the supervisor, perform rapid flu and strep tests, mono testing, pregnancy testing and dipstick urinalysis according to established procedures. Perform quality control procedures and proficiency testing accurately and as closely as possible to patient testing. Call critical results to the appropriate licensed caregiver and chart reports as needed.</li>
Depending on location may be required to perform electrocardiogram tracing ten lead and pediatric EKGs. Communicates critical results to authorized personnel<li>-Only at legacy Rockford locations</li><li>
Perform result entry in the LIS.</li><li>
Performs temperature checks.</li><li>
Cleans and checks laboratory glassware.</li><li>
Assists with inventory and maintenance of supplies at an appropriate level Submits orders for supply acquisition; receives and stocks supplies. Fills supply requests from clients and providers.</li><li>
Assists with orientation and training of new partners; coach in customer service excellence on an ongoing basis; assures competence of new partners prior to release of partner from training program; provide input for performance evaluations for phlebotomy partners to phlebotomy supervisor.</li><li>
Provides mentorship to phlebotomy staff as needed.</li><li>
Works collaboratively with supervisor to ensure adequate staffing and skill mix.</li><li>
Evaluate new products as necessary; provide written documentation of evaluation activities as required by regulatory agencies.</li><li>
Demonstrate exemplary phlebotomy skills with all types of patients; Use these skills to serve as a resource to the phlebotomy team.</li><li>
Takes ownership of the department; Identifies areas for improvement and makes suggestions to Phlebotomy Supervisor; Possesses ability to resolve issues.</li></ul>
EDUCATION AND/OR EXPERIENCE
High school diploma or equivalent preferred
Successful completion of a phlebotomy course or previous phlebotomy experience preferred.
Experience as a Paramedic, EMT, CNA or MA helpful.
CERTIFICATION/LICENSURE
Phlebotomy Certification preferred
