Summary:
Accurately codes and abstracts outpatient medical records for reimbursement and statistical purposes using established coding guidelines. Reviews coding and amends coding edits to assure compliance with all applicable regulations.
Responsibilities:<ul><li>
Codes all outpatient medical records in a timely and accurate manner according to department policy.</li><li>
Defines and transforms verbal descriptions of diseases, injuries, and procedures into numerical designations (codes) using ICD-10-CM and CPT-4 according to established coding guidelines.</li><li>
Initiates a physician/department query when there is conflicting, incomplete, or ambiguous documentation in the record or additional information is needed for accurate coding.</li><li>
Enters all required information accurately into computer system for reimbursement and statistical purposes. As applicable based on facility workflow, independently reconcile charges for areas of responsibility. Uses patient schedule together with billing slips to identify missing charges. researches and resolves discrepancy so charge keyed reflect services delivered.</li><li>
Performs ICD-10-CM diagnostic and CPT-4 coding at a minimum accuracy rate of 95%.</li><li>
Remains abreast of all applicable Federal, State, regulatory and hospital-specific coding guidelines.</li><li>
Applies applicable guidelines to all cases coded to ensure accuracy of selected codes.</li><li>
Accesses and research applicable reference materials to further support decision-making in code selection.</li><li>
Participates in Performance Improvement/Quality Assurance activities.</li><li>
Reports on software and hardware problems.</li><li>
Attends required educational sessions (webinars, conferences etc.) to maintain and enhance coding certification(s)</li><li>

Performs patient care under the physician's/nurse's direct supervision. Duties include preparing patient for physician, administering, obtaining and recording diagnostic laboratory test results. Assists in coordinating organization of care to meet patient needs including preparing, organizing and filing reports into the medical record.

Job Responsibility

<div><ul><li>Collaborates with members of the health care team in coordinating and implementing plans for patient care; may record reason for visit and related patient information for practitioner review.</li></ul>
<ul><li>Performs vital signs, height, weight, and screening procedures which may include head circumference, audiometry, and vision testing; documents findings in the medical record.</li></ul>

Summary:
Facilitates patient flow through the system from scheduling through registration check in so that patients have a &quot;one touch&quot; registration/scheduling experience. Responsible for obtaining demographic and financial information to ensure accurate patient identification and identify and secure appropriate payment sources. Responsibilities:

Performs insurance eligibility verification and executes payor requirements as needed. Includes initiating eligibility transactions via the internet, contacting the payor to obtain billing information and authorization and scripts for service when appropriate. 7. Provides estimates for services when appropriate, communicates and actively solicits patient liability. 8. Enters testing orders in the appropriate system when necessary. Requires an understanding of coding, procedural protocols, and the charge description master. 9. Summary: 
Provides clinical and clerical support to assure smooth daily department operations in assigned medical practice or practices. Assures that the patient and examination area is properly prepared.
Responsibilities: 
 1. Greets and brings patient back from waiting room to clinical area. Performs all necessary intake activities and documents appropriate information including vital signs, current medications and allergies, verifies EMR reflects current PCP, appropriate referring provider, and preferred pharmacy, etc.
2. Transcribes and updates patient information in the patient record. Transcribes demographic, medication history, social and medical history as reported by the patient from the intake form.
3. Provides clerical and secretarial work associated with patient care. Prepares and organizes patient charts for assigned providers. Completes other paperwork such as disability paperwork and scanning of other pertinent patient information as appropriate.
4. Performs and stays current with EMR Orders Reconciliation process as assigned.
5. Assures the examination area is clean after each patient use and stocked. Performs environmental rounds on a weekly/monthly basis as assigned.
6. Assures the patient is properly prepared for the scheduled visit to maintain timely patient flow. This includes assisting patients into gowns, having clinical paperwork, any needed supplies and educational materials immediately available for the provider use.
7. Conducts in-office testing as directed by the provider. Coordinates testing and outside ancillary services as applicable. Reviews instructions for upcoming procedures and tests as applicable.
8. Collects, prepares and processes specimens as directed by the provider. Completes process by assuring specimen results are logged in the accession book and are reconciled in the patients record.
9. Maintains inventory and ensures proper upkeep, cleaning and testing of medical equipment and supplies. Fills out appropriate logs.
10. Coaches and educates colleagues in office regarding procedures and techniques as new members join the group.
11. Performs pathology and other audits as assigned by supervisor.
12. Demonstrates and maintains practice-based competencies for in-office surgical procedures if applicable. This includes procedure setup, patient prepping with sterile technique, patient recovery and room turnover under RN direction.

Summary:
Provides critical care support to the nursing staff in all areas of the hospital to include transport of critically ill patients, member of the code team, rapid response team, and acute stroke team. Educates staff and provides team member development. Reports to Patient Care Director.
