The RN Case Manager - Home Health is responsible for the planning, coordination and delivery of quality home health care and services, ensuring compliance in accordance with company standards and federal, state, and local standards, guidelines, and regulations.
Essential Activities and Tasks
Care Management and Service Delivery - 70%<ul><li>
Provides ongoing assessment and nursing care to patient in homes and health care facilities. Ensures an accurate evaluation of patient needs, develops and implements a comprehensive care plan that addresses current needs. For home health, identifies when patients need or qualify for additional services and notifies appropriate team member or team leader.</li><li>
Follows current nursing professional standards of practice and agency approved protocols for home health.</li><li>
Provides quality plan and symptom management for the comfort of patients.</li><li>
Communicates with other health care providers about changes in condition and patient needs.</li><li>
Assesses patient and caregiver needs and provides education when necessary.</li><li>
Commitment to high quality customer service for internal and external customers.</li><li>
Oversees the implementation of the patient&rsquo;s plan of care. Evaluates the effectiveness of the plan on an ongoing basis.</li></ul>
Quality and Compliance Management - 30%<ul><li>
Audits and maintains current and accurate patient records through use of laptop computers and paper/chart documentation in accordance with policies, federal, state, and local guidelines.</li><li>
Participates in quality improvement projects as needed.</li><li>
Maintains knowledge of requirements of regulatory agencies, accrediting bodies, and third party payers.</li></ul>
All other duties as assigned.
Qualifications
Education<ul><li>
Current unencumbered license for the state of Ohio to practice as a Registered Nurse (RN) required.</li></ul>
Experience<ul><li>
One year experience in acute care or medical-surgical nursing required.</li><li>
Experience in a Medicare-certified home health agency preferred.</li><li>
Proficiency with Windows, Microsoft Office (Word, Excel, PowerPoint), and the internet required.</li><li>
Proficiency with electronic medical records (EMR) preferred.</li></ul>
Other Requirements<ul><li>
Must be able to read, write, speak, and understand the English language.</li><li>
Must possess a valid driver's license and acceptable driving record. Must be insurable under professional liability and crime coverage policies as specified by insurance carrier underwriting standards.</li></ul>

The Maintenance Associate performs the day-to-day activities of the Maintenance Department in accordance with company standards and federal, state, and local standards, guidelines, and regulations governing the life plan community to ensure that it is maintained in a clean, safe, and comfortable manner.
Essential Activities and Tasks
Operations Management - 80%<ul><li>
Performs routine HVAC, electrical, mechanical, plumbing, carpentry, painting and finishing, and maintenance on facilities, equipment, grounds, parking areas, sidewalks, etc.<ul><li>
Services HVAC equipment through preventative maintenance schedules and maintenance cycles.</li><li>
Troubleshoots, provides maintenance, and repairs the technical components of the community including HVAC, pumps, motors, etc.</li><li>
Troubleshoots, provides maintenance, and repairs electrical systems per NEC guidelines.</li><li>
Troubleshoots, provides maintenance, and repairs plumbing systems per local codes.
</li></ul></li><li>Informs supervisor of supply and/or equipment needs.</li><li>
Performs unscheduled maintenance tasks.</li><li>
Completes assigned work orders within 24-48 hours.</li><li>
Completes work orders related to survey readiness.</li><li>
Ensures a clean and safe environment for the residents, staff members, and visitors in all areas of the community by escorting vendors, assisting with inventory, conducting training, and serving as a working team leader as needed.</li><li>
Serves as back-up to security staff and responds accordingly performing necessary rounds and completing reports as needed.</li></ul>
Quality, Compliance, and Risk Management - 15%<ul><li>
Ensures that work areas are clean and that equipment, tools, supplies, etc. are in proper working order and are stored appropriately.</li><li>
Ensures that established quality assurance, infection control, and universal precaution practices are maintained when performing maintenance tasks</li><li>
Performs duties in accordance with all safety and OSHA requirements.</li><li>
Maintains current and accurate records through use of computers and/or other documentation in accordance with company standards and federal, state, and local standards, guidelines, and regulations.</li><li>
Willingly and consistently shares observations and insights of departmental operations that may improve quality and efficiencies of department.</li><li>
Attends all-employee and departmental meetings and in-service training as scheduled or as directed.</li></ul>
Customer Relations and Service Delivery - 5%<ul><li>
Listens and responds to the needs of residents and staff members.</li><li>
Honors resident personal and property rights when performing maintenance duties.</li></ul>

Qualifications
Education<ul><li>
High school diploma or equivalent required.</li><li>
Current certification to handle refrigerants a plus but not required.</li><li>
Current HVAC certification a plus but not required.</li><li>
Current Boiler Operator License and Steam Engineer License a plus but not required.</li></ul>
Experience<ul><li>
One year experience in a maintenance position required.</li><li>
Strong knowledge of HVAC, boilers, compressors and generators, and various mechanical, electrical, and plumbing systems.</li><li>
Knowledge of building codes, safety, EPA, and OSHA regulations preferred.</li><li>
Experience reading and interpreting blueprints preferred.</li><li>
Proficiency with Windows, Microsoft Office (Word, Excel, PowerPoint), and the internet preferred.</li></ul>
Other Requirements<ul><li>

The Executive Assistant and Compliance Coordinator provides support to the Chief Executive Officer and the Chief Human Resources &amp; Ethics Officer by being a proactive planner and trusted support. The Executive and Compliance Coordinator acts as a liaison for communications, scheduling, and special projects so the officers can stay organized, informed, and prepared to successfully deliver on commitments and effectively execute organizational operations in a timely and efficient manner.
Essential Activities and Tasks
Operations Management and Support - 40%<ul><li>
Proactively provides administrative support anticipating needs related to scheduling, meeting materials, communications, clerical support, special projects, event planning and coordinating, and distributing documents and other mailings.</li><li>
Multi-tasks as a shared support resource for two key executives effectively anticipating and fulfilling administrative and communications support activities.</li><li>
Manages detailed calendar activity requiring interaction with internal and external executives and assistants, as well as board members, consultants, or vendors.</li><li>
Coordinates and executes department, committee, board, and other meetings including room reservations, meeting notifications, agenda development, set up, and clean up (as needed).</li><li>
Makes travel arrangements.</li><li>
Prepares memos, letters, reports, slide presentations, handouts, flyers, etc. as requested.</li><li>
Handles collection and distribution of departmental mail and courier deliveries.</li><li>
Supports board, staff, resident, and/or patient events as requested.</li><li>

Adheres to records management system policy and procedures.</li><li>

Board Relations - 25%<ul><li>
Coordinates all aspects of board and committee meetings, including scheduling, agenda development, preparing meeting materials and presentations, facilitating mailings, set-up and clean-up (as needed). Manages minutes and attachments through the Board Portal.</li><li>
Serves as support for board and committee members and as a liaison to organization resources and staff members.</li></ul>
Customer Relations and Service Delivery - 20%<ul><li>
Supports the officers&rsquo; day-to-day relationships by interfacing with board members, locations, staff, residents, patients, and family members as a key administrative contact and liaison by answering questions, providing information, and ensuring appropriate follow-through and/or resolution is executed in a pleasant, professional, and timely manner.</li><li>
Reviews and responds to compliments, complaints, and grievances from residents, patients, staff members, and other constituents as directed by the supervisor.</li></ul>
Financial, Assets, and Materials Management - 15%<ul><li>
Processes check requests and pays invoices and expenses.</li><li>
Collects forms from new vendors used to add them to the accounting system.</li><li>
Monitors departmental expenses placed on credit cards and P-cards.</li><li>
Records budgetary expense information.</li><li>
May assign purchase order (PO) numbers to PO request, notify staff of the PO status, make PO adjustments, as necessary, and file paperwork appropriately, if applicable.</li></ul>

Qualifications
Education<ul><li>
Bachelor&rsquo;s degree in business administration or a related field.</li></ul>
Experience<ul><li>
Five years experience in a professional office setting in an administrative/clerical role supporting a department and/or senior management.</li><li>
Experience in scheduling travel preferred.</li><li>
Must be able to type a minimum of 75 words per minute.</li><li>
Proficiency with Windows, Microsoft Office (Word, Excel, PowerPoint), and the internet required.</li></ul>
Other Requirements<ul><li>
Must be able to read, write, speak, and understand the English language.</li></ul>
