Provides proper transportation techniques to ensure the efficient transport of patients in a safe and professional manner.</li></ul></li><li>
Responsibilities:</li><li><ul><li>
Performs patient transportation services using stretcher, wheelchair, wheeled-bed, or morgue stretcher following specific instructions of transportation dispatcher, nurse, or Administrative Coordinator.</li><li>
Responsible for pick-up and delivery of patient's chart to and from unit with each patient transport.</li><li>
Assists nursing staff in transferring patient from bed, wheelchair, or stretcher, and assists in patient ambulation upon request.</li><li>
Keeps log of daily assignments and accurately documents patient transport activity on the electronic medical record.</li><li>
Checks patient identification to assure transport of correct patients 100% of the time. Reports noted discrepancies to nurse in charge immediately, prior to transport.</li><li>
Reports any problems or difficulties encountered in the performance of their duties immediately to Transportation Dispatcher and documenting these difficulties.</li><li>
Reports any problems with patients to nurse, Staffing Office Supervisor, and/or Dispatcher.</li><li>
Responsible for properly cleaning transportation equipment following a patient transport. Responsible for maintaining patient transportation equipment and reporting defective equipment</li><li>
Assists the Staffing Office Assistant in the orientation of new employees as requested.</li><li>
Uses safety measures such as; side rails raised, patient positioned properly, etc., while transporting patients. Maintains safety measures during all transports.</li><li>
Assists funeral directors with the removal of bodies from the morgue as requested.</li><li>
Assists with the movement, storage, and monitoring of oxygen tanks within the hospital and during patient transports.</li><li>

Summary: 
<ul><li>
Primary duty is to transport patients throughout the medical complex in a safe, courteous, and timely manner. When appropriate and requested by clinical personnel the Telemetry Transporter will monitor the patient's vital signs including heart rate, blood pressure, respiratory rate, lung sounds, VAD parameters, pulse oximetry, and ECG tracings via appropriate monitor. In addition, the Telemetry Transporter will continuously assess the patient's overall condition and immediately notify the patient's nurse if any adverse conditions are detected or observed. When necessary, Telemetry Transporter Initiate Rapid Response and Code Calls for patients in declining status. Responds to assigned clinical emergencies and partakes in the hospital emergency preparedness plan for internal and external emergency management. Assists with patient movement when transferring patients from bed to wheelchair or stretcher. Carry out minor patient care duties while the patient is in transit. Transports deceased patients to the hospital morgue. Updates appropriate transport tracking system through use of assigned cell phone to maintain accurate location and times associated with patient transports.</li></ul>
Responsibilities: 
<ul><li>
Ensures all requirements are met in order to maintain active EMT and CPR certification.</li><li>
All transport assignments should be responded to in a safe and timely manner. Throughout the medical center, transporter maintains a constant focus on the safety and well-being of any patient in their care. Proper techniques and equipment should be utilized to facilitate safe, efficient and timely movement of patients.</li><li>
Follows hospital established protocols/policies and procedures when verifying patient identity. Ensures &ldquo;Ticket to Ride&rdquo; is updated accordingly, always including date, time and signature of Transporter and RN. Receives and relays pertinent patient information to Nursing/Provider staff at the start and completion of every transport and at every appropriate hand-off point 
completing all required documentation. Maintains patient's chart during transport and ensures confidentiality of patient information at all times.</li><li>
Utilizes &ldquo;Safe Patient Handling&rdquo; techniques when transferring patients to wheelchairs, stretchers, beds, and procedure tables. Assists hospital staff with the movement of patients as needed.</li><li>
Secures patient and any appropriate equipment prior to lifting, moving or transporting. When required assures adequate oxygen tank, IVs, drainage tubes, etc. are available prior to start of transport and upon arrival to destination. Connects patient to appropriate monitoring devices prior to and upon completion of every telemetry transport.</li><li>
Immediately alerts care providers, as well as the Dispatcher, Shift Supervisor/Manager/Director to any signs of patient distress, equipment malfunction, and unsafe conditions</li><li>
Clocks in and out of E*Star on time and promptly logs into appropriate transport tracking system within the department designated timeframe of scheduled shift time. When necessary advises dispatcher of availability in a timely manner. Acknowledges receipt of assignment, updates whereabouts immediately upon arrival and exit from location or destination and acknowledges completion of assignments in real time.</li><li>
Informs patient of any pending actions prior to attaching monitoring equipment including BP Cuffs, Monitor Leads, Pulse Ox, etc.</li><li>
When not assigned to active transports utilize downtime to check and properly store Department equipment.</li><li>
Follows hospital established protocols/policies and procedures for isolations precautions, hand hygiene and Personal Protective Equipment.</li><li>
Obtains passing score on annual dysrhythmia recertification exam.</li></ul>

The Clinical Nurse 2 Colleague independently provides patient care through the application of the nursing process and accepts accountability for the nursing care of assigned patients. Assigned patients may range in age from infancy to the elderly. The Clinical Nurse 2 Colleague demonstrates within the competency domains of Continuous Quality Improvement, Evidence Based Practice and Research, Leadership, Person and Family Centered Care, Professionalism, Safety, Technology/Informatics, and Teamwork (Definitions listed below). The Clinical Nurse 2 Colleague works closely with the inter-professional health care team to facilitate the coordination of care across the continuum. The nurse focuses on expanding the knowledge and skills necessary to provide individualized care based on physical, psychosocial, cultural, educational, safety, and age appropriate considerations of assigned patients. The Clinical Nurse 2 Colleague seeks feedback in assuming leadership roles and provides feedback for improved clinical practice. The nurse demonstrates how nursing practice impacts the organizational vision, mission, and goals and the care delivery system. The nurse complies with all regulations and standards of regulatory and accrediting bodies.</li></ul>
Responsibilities: 
<ul><li>
Clinical Practice: Provides age appropriate care for the population served. Gives consideration to growth &amp; developmental phase of life, to age related factors such as thermal regulation, nutrition, medication administration, nutrition, safety and functional needs of the patient. Uses appropriate resources for patients with special needs such as language barriers, sensory problems, altered cognition, cultural and religious needs. Ensures attention to patient&rsquo;s physical, spiritual and emotional well being. Demonstrates competency in assessment skills (Includes admission and shift assessments). Performs pain assessments and intervenes to optimally meet patient&rsquo;s needs for comfort. Plans clinical care based on patient&rsquo;s needs and uses available resources. Performs clinical care in accordance with policy and procedure. Performs peripheral venipuncture and peripheral and central vascular access care according to policy &amp; procedure. Evaluates outcomes of nursing interventions and takes appropriate actions to move patient toward reaching goals of care. Identifies situations that warrant patient rescue and takes appropriate action, e.g. Rapid Response or Code Blue/Pink. Adheres to the 5 rights of medication administration and uses the Medication Administration Check (MAK) system per policy and procedure. Integrates principles of infection control &amp; infection prevention into practice. Adheres to aseptic technique as appropriate. Uses resources and references for reviewing policies and procedures, medications, and clinical information. Prioritizes and organizes work effectively within assigned shift. Adheres to CCH Joint Commission Disease specific guidelines and core measures for the care of the patients with CVA Performs peritoneal dialysis according to policy and procedure. Performs med surg RN responsibilities for the patient requiring hemodialysis. Implements the nursing process for the adult and senior adult patient who presents with complex medical needs.</li></ul>

Education Training and Experience
Bachelors of Science in Nursing (BSN) is required.
More than 15 months relevant professional nursing experience and completion of the Nurse Residency Program, if applicable.
Basic computer skills: keyboard, mouse use. Ability to complete data entry on clinical documentation screens. Ability to use email and internet. Ability to successfully complete on the job training for CCH computer programs. Ability to practice critical thinking and organizational skills .Ability to speak, read, write and understand English. 
Licensure/Certification
Current Pennsylvania RN License.
CPR certification required. ACLS certification required, or must complete within 12 months of hire. Certification in clinical specialty is preferred.
