The Facility Based Crisis (FBC) Center is a community-based facility that serves as an alternative to inpatient hospitalization when appropriate. The FBC is a 16-bed facility operating 24/7/365 for adults aged 18 and over experiencing behavioral, emotional, psychological, detox, or other acute crisis stabilization needs. The FBC provides short-term intensive evaluation, treatment intervention or behavioral management to stabilize acute or crisis situations:</div><div><ul><li>
Assessment and evaluation</li></ul></div><div><ul><li>
Intensive treatment and stabilization</li></ul></div><div><ul><li>
Linkage to services that will provide further treatment or rehabilitation upon release.</li></ul></div>
The FBC Nurse is a member of a team that provides support, services, and treatments necessary to provide integrated care to individuals with behavioral health diagnosis or substance use disorders. Provides oversight and general nursing care and services to persons in a facility-based crisis/non-hospital detox setting. All patients are able to perform their own ADL&rsquo;s. No physical or chemical restraints. Average length of stay of patients is 5 -7 days.</div></div></div></div>
Pay: $36.00 per hour
Schedule:

</li><li>Will consider every other Sunday.
</li><li>PRN shifts available.</li></ul><ul><li>Must be willing to work every other weekend</li></ul>

Job Responsibilities<ul><li>
Utilize your skills and expertise, current nursing assessment skills and knowledge of nursing principles to assess and monitor clients, participating in behavioral health and substance use disorder treatment and responding to crises on a timely basis.</li><li>
Comply with Controlled Substances policy and procedures in maintaining accurate inventories and controlled logs, following appropriate guidelines for investigation of discrepancies</li><li>
Monitor consumers and taking appropriate action for side effects and adverse reactions to medications</li><li>
Complete QA activities as requested and participating in the QA process</li><li>
Communicate effectively with the team</li><li>
Our ideal Registered Nurse has excellent listening and communication skills, can pick up on nonverbal communication and is eager to work on nonstandard nursing assignments.</li></ul>
Education, Licensure, and Experience required for the position include:<ul><li>
Must be a graduate from an accredited school of nursing, with a minimum of an Associates Degree in Nursing.</li><li>
Must meet the education, experience, licensure/certification requirements specified by the service definition of the service being provided</li><li>
Prefer one year of nursing experience.</li><li>
Experience in behavioral health nursing preferred.</li><li>
Some college training and/or management experience helpful</li><li>
Valid drivers license (this is a condition of employment) including personal vehicle insurance coverage.</li><li>
Must maintain proficiency in company sponsored training and certifications and privileging if required.</li></ul>

Job Responsibilities<ul><li>
Provide holistic nursing care to individuals with intellectual and developmental disabilities</li><li>
Assist the Registered Nurse in delivering quality nursing services to persons supported</li><li>
Provide health related training to unlicensed personnel</li><li>
Administer medications and treatments or assist the person supported with medication administration</li><li>
Assist with ensuring preventative and routine medical appointments are scheduled</li><li>
Conduct medication closest assessments</li><li>
Collect health related data</li><li>
Observe for and report any abnormal health related findings to the physician</li><li>
Communicate with physicians, pharmacies, medical suppliers, and the Interdisciplinary Team</li><li>
Advocate for the people supported and assist with developing community supports to promote optimal health for the people supported</li></ul> 
Requirements<ul><li>
Graduation from an accredited licensed nurse program</li><li>
Current nursing license with no restrictions to practice nursing.</li><li>
Current CPR Certification or eligibility to obtain certification</li><li>
Must be able to communicate effectively with Interdisciplinary Team Members, as well as the people supported and their families</li><li>
Must be able to work independently, be organized, and possess the ability to prioritize work tasks</li></ul>
Physical requirements include: ability to regularly lift 20 pounds. Must be able to lift a minimum of 40 pounds. Must be able to pull a minimum of 25 pounds. Must be able to squat, kneel, crawl, crouch, climb, and stoop. Must be able to regularly stand and walk. Must be able to demonstrate proficiency in CPR from the floor level. Vision requirements include close vision, distance vision, and peripheral vision. Must be able to communicate.

Reporting client&rsquo;s social, psychological and physical behavior as indicated</li><li>
Implementing and ensuring compliance with standards of nursing practice that promote optimal health care delivery and meet regulatory requirements</li><li>
Implementing policies and procedures to ensure organizational compliance with Confidentiality regulations, HIPAA (Health Insurance Portability and Accountability Act), national accreditation agency and Continuous Quality Improvement, attending applicable meetings as necessary</li><li>
Using state guidelines, Quality Assurance Plan and Client Rights Policy, ensuring that clinical operations comply with state regulations and update plans and policies as necessary</li><li>
Supervisory responsibilities include interviewing, making recommendations for hiring, and training employees; planning, assigning and directing work; appraising performance; rewarding, motivating, disciplining and promoting team performance; addressing complaints and resolving problems</li></ul>
Requirement<ul><li>
Active and current LPN Licensure in state of employment</li><li>
2 years&rsquo; experience as a nurse working with behavioral health clients, preferred</li><li>
Working knowledge of psychoactive substance abuse, dependency and the principles and techniques based on best practices, commitment to the use of evidence based treatment and preventative methods</li><li>
Valid driver&rsquo;s license and automotive insurance required</li></ul>
