Job Summary</b></p><p>
We are seeking a Licensed Vocational Nurse (LVN) Clinical Coordinator to support home health intake, care coordination, and clinical operations. This role ensures timely admissions, regulatory compliance, and high-quality patient care.</p><p></p><p><b>
Key Responsibilities</b></p><ul><li><p>
Manage patient intake, referrals, and admissions</p></li><li><p>
Review documentation for home health eligibility</p></li><li><p>
Coordinate care with physicians, patients, and families</p></li><li><p>
Ensure compliance with Medicare, state, and Joint Commission regulations</p></li><li><p>
Track hospitalizations, holds, and start/resumption of care (SOC/ROC)</p></li><li><p>
Participate in IDT meetings and QAPI initiatives</p></li><li><p>
Support field staff with clinical questions and urgent needs</p></li></ul><p></p><p><b>
Qualifications</b></p><ul><li><p>
Active LVN license</p></li><li><p>
3+ years nursing experience (some home health required)</p></li><li><p>
Knowledge of home health, intake, or care coordination</p></li><li><p>
Strong communication and organizational skills</p></li></ul></div><p></p><p><b>
