PATIENT CARE TECH I

<div><div>Job Summary
Serves patients by preparing medications; giving pharmacological information to multidisciplinary health care team; monitoring patient drug therapies.
Performance: Position Specific Essential Functions
<ul><li>Dispenses medications and IV medications for patients ranging in age from neonatal through elderly adults in accordance with local, state, and federal laws and hospital policies and procedures.</li>
<li>Interprets medication orders and transcribes into the computerized patient medication record accurately.</li>
<li>Ensures safe, appropriate, cost-effective medication therapies for patients according to established policies, procedures, and protocols</li>
<li>Participates in the orientation of all new employees; provides in-services education to pharmacy and hospital staff; assists in preparing performance evaluations as required.</li>
<li>Assures accurate and complete labeling of drugs distributed from pharmacy; maintains records of transactions to assure control and accountability for all drugs.</li>
<li>Participates in quality improvement and medication use reviews in accordance with department and medical staff approved criteria.</li>
<li>Sustains the formulary by minimizing non-formulary procurement, utilizing therapeutic substitution protocols, and promoting rational drug therapy selection.</li>
<li>Provides clinical consultations and clarification to practitioners; suggests appropriate, cost-effective therapeutic alternatives to medical staff, as needed.</li>
<li>Documents all clinical activities and interventions accurately and completely.</li>

Pharmacist

<div>Job Summary
Responsible for the delivery of patient care as directed by the clinic provider. The medical assistant works closely with the clinic patient care manager, other professional staff, clerical staff, ancillary department members, visitors and families.
Responsible for basic nursing-related and medical office tasks of average difficulty.
