Do you have a passion for engaging with people and helping them on their journey to better health? Signify Health, part of CVS Health&reg;, is seeking a Full-time Clinician (Nurse Practitioner or Physician Assistant) to provide In-Home Health Evaluations, engaging with people in the comfort of their own homes and helping bridge gaps in care.
You&rsquo;ll play a critical role in building trusted relationships to make people healthier, helping bring flexible, comprehensive and personalized health evaluations right to people&rsquo;s front doors.
Job highlights
The visit, which lasts about 45 minutes, allows for time to connect one-on-one and answer health questions. It includes a medication and medical history review, a physical evaluation, and, if ordered by the person&rsquo;s health plan, additional diagnostic tests (i.e., diabetic eye exams, spirometry, etc.). You&rsquo;ll use an iPad and an intuitive clinical workflow for each evaluation and are not required to prescribe medicine, order lab tests or alter people&rsquo;s current treatment regimen.

Work with Signify Health as a clinician colleague, fulfilling the requirements of the specified role</li><li>
Be flexible to travel locally and within licensed states, as needed</li><li>
Conduct virtual visits as needed</li><li>
Provide all Signify Health services as indicated including Diagnostic &amp; Preventive Services products (i.e., spirometry, DEE, etc.)</li></ul>
A Note on Our Work Environment:
As an in-home clinician, your primary workplace is the private residence of health plan members. We are dedicated to meeting health plan members where they are, wherever they call home. These environments are diverse and may present a variety of conditions. Candidates must be able to comfortably and safely perform their duties in homes that may contain household pets, secondhand smoke and other potential airborne allergens or irritants.

The typical pay range for this role is:
$95,738.00 - $206,206.00
This pay range represents the base hourly rate or base annual full-time salary for all positions in the job grade within which this position falls. The actual base salary offer will depend on a variety of factors including experience, education, geography and other relevant factors.

In-Home Health - Nurse Practitioner or Physician Assistant (Full Time) - Cambridge MA

Signify Health, part of CVS Health&reg;, is hiring full-time, board-certified nurse practitioners or physician assistants to conduct Video Visits for Medicare Advantage, Medicaid, and certain commercial plan members. During our Video Visits, you&rsquo;ll conduct an approximately 30-minute visit, including a medication and medical history review, allowing time to educate members about their health and answer their questions.
In this role, you will:<ul><li>
Conduct online &ldquo;Video Visits&rdquo; with health plan members, working in a private space with adequate internet connection</li><li>
Provide 40 hours of availability weekly, consistently providing 320+ capacity units per month</li><li>
Execute a strong visit completion rate, consistently pacing 220+ completed visits per month</li><li>
Maintain training, affiliation, and compliance with Video Visit requirements (regular training required)</li></ul>
Other role expectations and requirements:<ul><li>
Be trained and credentialed for all health plan clients and lines of business in primary coverage state(s)</li><li>
Consistently engage with regular manager, team and clinical meetings</li><li>
Be accountable to Signify Health operational, clinical, and member experience key performance indicators and metrics</li><li>
Maintain active state licenses in coverage areas (FNP, AGNP certification preferred for NPs)</li><li>
Maintain board certification</li><li>
Primary coverage areas include but are not limited to: Colorado</li><li>
Experience with Signify Health In-Home Health Evaluations, Video Visits, in-home visits, telehealth visit delivery, or a similar format (preferred)</li><li>
Able to obtain and maintain multi-state licenses in coverage area, as required</li></ul>
Additional Required Skills, Abilities and Working Environment:<ul><li>
Clinicians will be dedicated to providing Video Visits (&ldquo;telehealth&rdquo;) services only</li><li>
Must meet minimum requirements for remote visit delivery, including broadband connectivity with video and audio enabled, a quiet setting with a neutral background from which to conduct visits, and the ability to uphold health plan member privacy per CVS Health guidelines.</li><li>
While performing the duties of the job, the employee is regularly required to interact with health plan members in a remote manner, sit, write, operate the computer and phone, speak intelligibly, and hear member responses</li><li>
Specific vision abilities include the ability to view and read a computer screen, iPad, and other electronic devices</li><li>
Effective verbal, written, and electronic communication skills</li><li>
Ability to work without direct supervision and practice autonomously</li><li>
Proficiency with information management and technology</li></ul>
