In this vital role, you will provide compassionate, high-quality patient care across various settings, including inpatient units, outpatient clinics, long-term care facilities, and community health environments. Your expertise will support patient recovery, comfort, and well-being while ensuring adherence to safety protocols and healthcare standards. This position offers an exciting opportunity to make a meaningful difference in the lives of diverse patient populations through active engagement and expert nursing skills.Duties<ul><li>Perform comprehensive patient assessments, including vital signs, physical examinations, and health histories to inform individualized care plans.</li><li>Administer medications and injections accurately following prescribed protocols, including IV infusion and injection administration.</li><li>Monitor patients&rsquo; conditions closely, utilizing medical terminology and physiology knowledge to identify changes requiring intervention.</li><li>Assist with medical procedures such as catheterization, suturing, tube feeding, and sterile processing techniques.</li><li>Document all patient interactions thoroughly using EMR (Electronic Medical Record) systems like Cerner or eClinicalWorks, ensuring compliance with HIPAA regulations.</li><li>Collaborate with interdisciplinary teams on discharge planning, case management, and post-acute care strategies to optimize patient outcomes.</li><li>Provide specialized care for individuals with dementia or Alzheimer's disease, including memory care and behavioral health support.</li><li>Support emergency situations with airway management, ventilator operation, and acute pain management in high-pressure environments such as ICU or Level I Trauma Centers.</li></ul>Skills<ul><li>Proven experience working with diverse patient populations in settings such as hospital medicine, critical care (ICU), emergency room (ER), or long-term care facilities.</li><li>Strong knowledge of ICD-9/10 coding systems for accurate medical documentation and billing processes.</li><li>Skilled in triage assessment and urgent care procedures to prioritize patient needs efficiently.</li><li>Experience with medical imaging procedures like sonography or laboratory specimen collection/processing is a plus.</li><li>Familiarity with EMR/EHR systems including Athenahealth or NCQA standards for seamless documentation workflows.</li><li>Expertise in infection control practices, aseptic techniques, and sterile processing to maintain patient safety standards.</li><li>Ability to work with individuals with disabilities or developmental delays in community or home care settings.</li><li>Competence in performing venipuncture (venipuncture), phlebotomy, catheter care, and basic life support (BLS).</li><li>Knowledge of anatomy and physiology essential for accurate physical examinations and diagnostic evaluation.</li><li>Excellent communication skills for patient education on health coaching topics such as nutrition or medication management.
