<li>Be a go-to support for your supervisor&mdash;check vitals, monitor skin health, and track weight changes.</li>
<li>Stay informed: review care plans daily and keep your team updated on any changes.</li>
<li>Be there for the important moments&mdash;help residents with admissions, transfers, and discharges.</li>

Qualifications:

<li>High school diploma/GED</li>
<li>Minimum 1 year of experience in a similar role</li>
<li>CPR/First Aid certification required</li>
<li>Completion of the Qualified Medication Aide program with a current license based on state requirements</li>

<li>Able to read, write, and verbally communicate in English</li></ul>

Nursing Assistant

As the Licensed Practical Nurse (LPN), you will be responsible for providing direct resident care and supervision of residents&rsquo; needs based on their individualized service/care plan. You will monitor residents for changes in condition and signs/symptoms of illness, in accordance with federal and state laws and regulations, as well as the policies of the Community. In partnership with the Health Services Director, you will communicate and document changes or concerns regarding residents and assist in supervising the care team.

 *****This is a Full-Time Day shift position*****

The successful candidate for this position will have:

<li>A passion for seniors and enhancing their quality of life</li>
<li>A valid Licensed Practical Nurse (LPN) license; a Compact license or the ability to obtain one is encouraged. An unencumbered LPN license is required.
</li><li>Minimum of at least one year of successful experience as an LPN in assisted living or long-term care</li>
<li>Experience administering medication and treatment plans per the doctor's orders and community policies</li>
<li>Knowledge of state regulations pertaining to resident care and understanding the importance of being state survey-ready every day.</li>
<li>Have experience working as a mandatory reporter and can immediately intervene, report, and investigate all suspected or alleged verbal, physical, sexual abuse, or theft from a resident.</li>
<li>Have a current driver&rsquo;s license.</li></ul>

The successful candidate for this position will have:

<li>A passion for seniors and enhancing their quality of life</li>
<li>A valid Registered Nurse (RN) license; Compact license or ability to obtain is encouraged. An unencumbered license is required. An LPN with leadership experience may also be considered.</li>
<li>Proven clinical leader in an assisted living or LTC environment with at least one year of successful experience as a Wellness Director, Health Care Coordinator (HCC) or Director of Nursing (DON)</li>
<li>Understand the importance of being state survey ready, every day</li>
<li>Have a current driver&rsquo;s license</li></ul> 


Essential Functions:
<ul>
<li>Be able to lead the clinical team with a sense of purpose and accountability; utilize metrics and established goals.</li>
<li>Oversees the medication management services, including training and audit of medication administration practices, collaboration with prescribing healthcare providers and pharmacy staff</li>
<li>Identifies ongoing needs and services of residents through assessment and service plans utilizing the nursing process</li>
<li>Visit prospective/current residents in their homes or healthcare settings as needed to assess for admission/readmission to the community.</li>
<li>Clinical liaison with families, residents, healthcare professionals, and team members</li>
<li>Direct the infection control measures for the community</li>
<li>Work collaboratively with others on the community leadership team</li>
<li>Manage and coordinate the work of the clinical staff. This includes overseeing and/or completing required training, staff schedules monthly and daily work assignments.</li>
<li>Provide on-call clinical support as needed.</li></ul>
